The subject of the contract is: the performance of the car sanitary transport services by the Contractor for the Health Care Team in Oświęcim in the field of round-the-clock transport sanitary, whereby sanitary transport is understood in accordance with art. 5 point 33 athe Act of 27.8.2004 on healthcare services financed from public funds (i.e., Journal of Laws of 2019, item 1373, as amended), hereinafter referred to as the Act on healthcare services, transport of persons to the Health Care Center in Oświęcim . The transport of people to the Health Care Center in Oświęcim must meet the requirements of the National Health Fund and include in particular the transport of Patient`s patients between departmentsbut due to the location in various buildings, to other medical entities, transport of patients for consultations, diagnostic tests and / or taking patients home and bringing the patient by the Contractor, transporting blood and material for diagnostic tests to other facilities, as part of s

The subject of the order is: performance of sanita car transport servicesby the Contractor for the Health Care Team in Oświęcim in the field of round-the-clock sanitary transport, whereby sanitary transport is understood in accordance with art. 5 point 33 a of the Act of 27.8.2004 on healthcare services financed from public funds (i.e. Journal of Laws 2019 item 1373, as amended), hereinafter referred to as the Health Care Services Act, for the Health Care Center in Oświęcim. The transport of people to the Health Care Center in Oświęcim must meet the requirements of the National Health Fund and include in particular the transport of the Ordering Party`s patients between departments due to the location in different buildings, to other medical entities, transport of patients for consultations, diagnostic tests and / or taking patients home and bringing (brought)e) the patient by the Contractor, blood transport and diagnostic examination material to other facilities, as part of his activity - by land transport to places indicated (e.g. apartment, clinics, hospital, etc.) by the Employer. The number of courses depends on the current needs of the Employer indicated in the Employer`s order. A detailed description of the subject of the contract and requirements in the scopeThe essence of its implementation is included in Annex 6 to the ToR.

The Employer states that the total estimated number of kilometers to be implemented for the benefit of the Employer (based on the last 12 months) is about 220, 000 km per year and the total estimated number of working hours to be implemented on the Employer`s benefit (based on the last 12 months) is about 8, 500 h per year.

Kompetefunctions or entitlements to conduct a specific professional activity, if it results from separate provisions. that the Contractor has a current entry in the Register of Entities Carrying out Medical Activities in accordance with the Act frommedicinal activity i.e. of September 8, 2016 (Journal of Laws of 2018, item 2190, as amended) Annex 3 to the ToR;

- the Contractor`s statement about the civil liability insurance policy, and in the absence of it, of having another confirmation documentthat the Contractor is insured in the field of civil liability in the scope of conducted activities related to the subject of the contract together with the obligation of the selected Contractor to provide such a document by the date of signing the contract for a guaranteed amount not less than resulting from the Regulation of the Minister of Finance on compulsory civil liability insuranceof the entity performing medical activity of 29.4.2019 (i.e. Journal of Laws 2019, item 866). Annex 3 to the ToR. shorter than 10 days, valid on the day of submitting the following statements or documents:

a) a valid excerpt from the relevant register, ifseparate provisions require an entry in the register in order to demonstrate the lack of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act, issued not earlier than 6 months before the deadline for submission of bids, and statements regarding natural persons in the scope of art. 24 paragraph 5 point 1 of the Act;

b) a current certificate of the competent head of the Tax Office confirming that the Contractor does not dependwith payment of taxes, or a certificate that he has obtained exemption provided for by law, deferment or installment of overdue payments or suspended in full implementation of the decision of the competent authority issued not earlier than 3 months before the deadline for submission of bids;

c) current certificate of the competent branch of the Insurance Company Społecznych or Kasa Rolniczego Ubezpieczenia Społconfirming that the Contractor is not in arrears with payment of health and social insurance contributions or confirmation that he has obtained the exemption provided for by law, deferment or installment of overdue payments or suspended in full implementation of the decision of the competent authority - issued not earlier than 3 months before the deadline for submission offers;

d) current entry in the Register of the entitythose carrying out medical activities in accordance with the Act on medical activities, i.e. of 8 September 2016 (Journal of Laws of 2018, item 2190, as amended);

e) a valid document confirming the entitlements for at least 10 persons in accordance with road traffic regulations suitable for driving the vehicle in which the service will be carried out, including at least 10 persons holding a driving licenseprivileged in accordance with the Act of 05 January 2011 - on drivers of vehicles (Journal of Laws of 2019 item 341 as amended) and the Regulation of the Minister of Transport, Construction and Maritime Economy of 15 May 2013 on courses for drivers emergency vehicles and vehicles carrying cash (i.e. Journal of Laws of 2018 item 1392);

f) current authorization of the Minister of the Interiorand the use of motor vehicles as privileged in road traffic in the case of using these vehicles in connection with saving lives or human health, referred to in art. 53 section 1 point 21 of the Act of 20 June 1997 Road Traffic Law (Journal of Laws of 2018, item 1990, as amended) to confirm compliance with the criterion "number of priority vehicles" in accordance with the submitted declaration;

g) documents confirming that the services indicated in the list in the statement (Annex No. 3 to the ToR) were performed in accordance with the contract;

h) list of persons confirming the professional capacity shown in the statement (Annex No. 3 to the ToR) in accordance with the model Annex No. 8 to the Terms of Reference, together with documents confirming the authorizations; i) list of sanitary vehicles confirming the technical capabilities shown in the statement (forł. No. 3 to the Terms of Reference) in accordance with the specimen of Annex No. 9 to the Terms of Reference;

j) documents confirming the stationing of car fleet (e.g. rental agreement) with the personnel at a distance appropriate to the declared time of arrival;

k) statement regarding the compliance of vehicles with the PN standard -EN 189: 2008 "Medical vehicles and their equipment road ambulances" with the Regulation of the Minister of Infrastructure of 31.12.2002 inon technical conditions of vehicles and their necessary scope (Journal of Laws of 2016, item 2022) - Annex No. 3a to the Terms of Reference

1. The offer must be secured by a deposit.

2. The value of the bid bond is PLN 46, 000.00 (say: forty six thousand zlotys 00/100)

3. The bid security is lodged before the bid submission deadline.
