Reference Number: 204 / PN / 2019

1. The subject of the order is the provision of the transport service insurance, in accordance with the offer and price form constituting Annex 5 to the ToR and the description of the subject of the order contained inAnnex No. 4 to the ToR.

Main Site: The seat of the Employer and the area indicated in the description of the subject of the order.

1. The object of the contract is to provide a belaying transport service, in accordance with the offer and price form constituting Annex 5 to the ToR and the description of the subject of the order contained in the Annex No. 4 to the ToR.

2. Requirements transport R, team and vehicle are set out in Annex 10 to SIWZ

3.Filled out Annexes 5 and 10 to the ToR, the Contractor is obliged to submit with the offer

4. The belay transport service will be carried out 7 days a week between 8.00-20.00, where 8.00 means the service start time and 20.00 means time of acceptance of the last order for implementation.

5. In urgent situations, outside of the above hours, the Contractor shallAny arrangement is to organize the patient`s transport, provided that it is necessary to transport it to a medical unit other than UCK.

6. The Contractor shall provide direct stationary and cellular communication with the Transport Dispatcher, to whom all transports will be ordered by phone, confirmed by a completed "Transport Order" patient "and will also provide permanent cellular communication withemergency ambulance service. 7. Additional requirements:

1) employees who perform the service depending on the needs of the Employer will perform the role of caring for the patient in the situation of ordered tests, specialist consultations, dialysis, transfer to another facility for further treatment, transport to home and will be responsible for the entrusted medical documentationpatient, including for secrecy pursuant to the Personal Data Protection Act.

2) The Contractor shall provide medical ambulance equipment in accordance with applicable standards and technical requirements

3) The Contractor shall ensure that patients can be transported lying down for one person and sitting for a minimum of two persons

4) Ambulance type "S" equipped with an oxygen source and oxygen system, nosefrom the type of cart and other necessary medical equipment specified by statute for this type of vehicles, in accordance with the Act on Emergency Medical Services and the ordinance of the President of the National Health Fund.

5) The Contractor undertakes to ensure cleanliness including proper disinfection in accordance with sanitary and epidemiological requirements and aesthetics of vehicles used to perform transport

6) The Contractor undertakesę to train employees providing the service in the scope of providing first aid and will submit relevant documents in this respect

7) The Contractor undertakes to provide personal identifiers to all employees providing the service with the name, surname, position and company name, as well as to provide a uniform work outfit .

8) The contractor is obliged to keep up to dateego informing about changes in the employment of staff and vehicles providing the service, subject to all the conditions specified in the procedure

9) The Contractor is obliged to undergo random checks by the Head of the Supply and Transport Department of the UCK and representatives of the National Health Fund

10) The Contractor will provide a list of vehicles with details equipment in the field of materials, equipmentand medical equipment as well as liability and accident insurance package for each vehicle.

11) The Employer does not agree to employ subcontractors who will act on behalf of the Contractor.

8. The detailed requirements and scope are included in Annexes 4, 5 and 10 to the ToR. This condition will be met if the Contractor demonstrates that he is entered in the register of entities performing medical activities, in accordance with art. 103 of the Act of 15 April 2011 on medicinal activities.

The condition will be pre-verified on the basis of completed Part IV: Qualification criteria by submitting a general statement regarding all qualification criteria in point α (alpha) in ONE

1. This condition will be met if the contractor proves that he is insured against civil liability inscope of activity related to the subject of the contract, i.e. the provision of sanitary transport services, a guarantee sum of PLN 1, 000, 000.00.

The condition will be pre-verified on the basis of completed Part IV: Qualification criteria by submitting a general statement regarding all qualification criteria in point α (alpha) in ONE

2. The contractor whose offer has beenthe highest rated in the said procedure, to confirm compliance with the conditions for participation in the procedure, at the request of the Employer, will submit the following documents:

1) to the extent indicated in paragraph V point 2 letter a) of the ToR, current entry in the register of entities performing medical activities,

2) To the extent indicated in par. V point 2 letter b) of the Terms of Reference documents confirming that the contractor is insuredliability insurance in the scope of conducted activities related to the subject of the contract for a guarantee sum of not less than PLN 1, 000, 000.00

1. The awarding entity does not envisage awarding the contracts referred to in art. 67 section 1 item 6 of the PPL Act

2. The Purchaser does not allow submission of variants.

3. The Employer allows the amendment of the contract in accordance with Annexes 3 to the ToR.

4. In the case oftowards the values provided by the Contractors on the statements and documents,

referred to in item VII of the ToR, will be given in a currency other than PLN, the Employer will convert these values to PLN assuming the average NBP exchange rate of the given currency as at the date of initiating the procedure.

5. All activities, powers and obligations of Contractors and Employers not regulated in these ToR, which the Act did not require to include Zthe applicant in the ToR, and which may contribute to the proper course of the proceedings, is regulated by the PPL Act

6. The contracting authority envisages making changes to the contract during its implementation in the event of circumstances referred to in art. 144. Public Procurement Law

7. information on the bid bond was included in section IX SIWZ.

8. Information on documents (including from foreign entities) is included in paragraphVII SIWZ.

9. Information on how to prepare the offer is included in paragraph XI SIWZ.
