Delivery with the assembly of solar collectors in the commune of Jedwabne

Reference Number: BIOŚ.271.13.2018

The subject of the order is the delivery together with the assembly of 118 sets of solar collectors in the commune of Jedwabne, specified in the detailed description of the subject of the order contained in Annex 2 to SIWZ.

Main Site: Municipality of Jedwabne

Delivery togetherwith the installation of 118 sets of solar collectors used to prepare domestic hot water in 118 residential buildings located in the commune of Jedwabne, including: a) solar installation type 1 with the use of 2 solar collectors and a hot water tank. o. 200 liters, 30 pcs.

B) solar installation type 2 using 3 solar collectors and a hot water tank. o. 300 liters 59-pcs.

c) type 3 solar installation using 4 solar collectors and a hot water tank. o. 400 liters, 23 pcs.

D) solar installation type 4 with 5 solar collectors and a hot water tank. o. 500 liters 5 pcs.

E) solar installation type 5 with 6 solar collectors and a hot water tank. o. 600 liters 1 each.

The ordering party does not place any specific conditions

Contractors who have financial resources or creditworthiness in the amount not less than PLN 700, 000.00 can apply for the order. Contractors who are not subject to exclusion from the procurement procedure can apply for the order. art. 24 sec. 1 point 12-23 of the Public Procurement Law (hereinafter the Public Procurement Law) and art. 24 sec. 5 points 1, 2 and 8-Public Procurement Laws. To confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion, the following statements and documents are used (in addition to the documents listed in Section III point 1.2.i and 1.3):

1. Uniform European Order Document - EAT [according to the Annex No. 3 to the ToR] Contractor may limit himself to filling only the section α within the scope of part IV of the form and he does not have to fill inany of the other sections of Part IV of the qualification criteria form,

2. information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

3. a copy from the relevant register or from the Central Register and Information on Economic Activity, if a separate provisiony require an entry in the register or records to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Public Procurement Law,

4. certificate of the competent head of the tax office confirming that the Contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has entered into an agreementnot with the competent tax authority regarding the repayment of such receivables, together with any interest or fines, in particular obtaining the legally required exemption, deferral or payment in installments of overdue payments or suspension in full of the implementation of the decision of the competent authority; certificate of the relevant local organizational unit of the Social Insurance Institution or Farmers' Fund of UbSocial Security or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding repayment of these receivables with any interest or fines, in particularhe has obtained the exemption, deferral or payment by installments of the overdue payments provided for by law, or suspension of the execution of the decision of the competent authority in full;

6. the Contractor's statement that he has not been the subject of a final court judgment or final administrative decision regarding his overdue payment of taxes, social security or health insurance premiums, or in such a casedecision or decision confirming the payment of these receivables together with any interest or fines or entering into a binding agreement regarding the repayment of these receivables [according to Annex 8 to the ToR]; 7. declaration of the Contractor about the lack of a ruling against him as a preventive measure of the prohibition to apply for a public order [according to Annex 8 to the ToR];

8. obliOther entities are obliged to provide the Contractor with the necessary resources for the needs of order fulfillment (applies to Contractors based on the capacity or situation of other entities pursuant to Article 22a paragraph 2 of the Public Procurement Law) according to Annex 7 to the ToR.

EAT should be sent in an electronic form qualified electronic signature. EAT should be sent to the e-mail address: burmistrz@jedwabne.pl before the deadline for submitting offers. The contracting authority allows in particular the following format of the transmitted data: .pdf, .doc, .docx, .rtf, .xps, .odt. The signed EID electronic document should be encrypted, i.e. with an access password. The Contractor shall place a password to access the file containing the EED in the content of its offer, submitted in writing
