The subject of the order is the delivery and assembly of solar collectors and photovoltaic installations in the commune of Wojciech-w, which is implemented as part of the project "Installation of solar collectors and solar panels on residential buildings in the area Wojciech-w commune.

Main Site: Commune areaWojciechow

Supply and installation of solar collectors on residential buildings located in the area of the Wojciech-w Commune, including, among others:

1) delivery and assembly based on the technical documentation of 171 sets of solar collector installations for domestic hot water preparation together with the structure adapted to the place of assembly, including:

a) 164 kpl (in composition1 set enter: 2 panels and 300 dm storage) mounted on the roofs and fa-ades of residential buildings, natural persons,

b) 7 sets (1 set includes: 3 panels and a 400 dm container) installed on the roofs and fa-ades of residential buildings, natural persons,

2) disassembly of the existing DHW heater and disconnecting it from the installation,

3) installation of a new hot water heater with mixing valve and integrated(4) installation and isolation of pipelines between collectors, pump group, and hot water heater,

5) installation of the pump group,

6) installation of temperature sensors in solar collectors and in the tank,

7) connection of the existing source of heat for hot water heater,

8) rinsing with solar liquid and carrying out solar installation tightness,

9) filling, airing and proper adjustment of liquid flow and determination of correct pressure according to the instructions of the solar producer,

10) finish at least according to the initial condition of the installation transition (plaster, insulation, passages through walls, ceilings, roof) and effective protection against the effects of atmospheric places outside the facility where it would be run(11) programming and commissioning of the automation system together with connection of the WLAN communication modem to the controller in order to read the energy yield parameters via the Internet for 15 solar collector installations. The controller saves the daily monthly and annual energy collected by the solar collectors on the microSD or SD card, and enables the transfer of savedinformation on the mobile external device. The controller or an additional module for communication with the network has a built-in WiFi based on radio communication for wireless connection to the local existing network (WLAN), and cooperation with the monitoring system,

12) training of the installation user to operate the devices used,

13) transmission of technical documentation start-up, operating instructionsand 14) other elements included in Annex 1a to the ToR (technical documentation) and the draft of the contract constituting Annex No. 2a to the ToR.

-6.1. Contractors who are not subject to exclusion and meet the conditions of participation in the procedure specified by the Employer may apply for the award of the contract,

-6.2. Contractors may apply for the award of the contractmeet the conditions regarding:

-6.2.1. competences or entitlements to conduct a specific professional activity, if it results from separate regulations: - the Ordering Party does not specify the condition in the above-mentioned range.

-7.1. The Contractor shall be excluded from the procurement procedure in relation to which any of the circumstances referred to in art. 24 sec. 1 point 12-23 of the PPL Act,

-7.2. ORDERINGwhich provides for the grounds for exclusion indicated in art. 24 sec. 5 points 1, 2, 4 and 8 of the Act,

-8.1. The Contractor is obliged to attach a statement valid as of the day of submitting bids constituting preliminary confirmation that the Contractor:

a) Is not subject to exclusion,

b) Meets the conditions for participation in the procedure

-8.2. The Contractor is obliged to submit the declaration referred to in point 8.1 of the Terms of Reference in a uniform forma document prepared in accordance with the standard form provided for in the Commission Implementing Regulation 2016/7 of 5.1.2016 issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU, hereinafter referred to as "Single Document" or "EAT" acc. Annex 4 to the ToR,

-8.3. Contractor within 3 days from the date of posting on the website of the contracting party information about whichreferred to in Article 86 par. 5 of the Act (information from the opening of tenders), is obliged to provide the Employer with a declaration of belonging or absence of belonging to the same capital group, referred to in art. 24 sec. 1 point 23 of the Act. In the case of being part of the same capital group, the Contractor may submit together with the declaration documents or information confirming that the relatedand with another Contractor do not lead to distortion of competition in the proceedings. The pattern of the declaration is attached as Annex 5 to the ToR,

-9.6. The Employer demands from the Contractor, which relies on the abilities or situation of other entities on the terms specified in art. 22a of the Public Procurement Law, present in respect of these entities the documents listed in point 8.7.2 SIWZ,

-9.8. The contractor, who relies on zasto prove the absence of grounds for exclusion and fulfillment to them, to the extent that they refer to their resources, the condition of participation in the proceedings also consists of EED regarding these entities,

-10.2. In the case of Contractors jointly applying for the awarding of orders:

a) None of them may be subject to exclusion due to non-compliance with the conditions referred to inand in art. 24 sec. 1 and par. 5 points 1, 2, 4, 8 of the Act on Public Procurement Law, while fulfillment of the conditions for participation in the Contractor's proceedings are shown in accordance with point 6.2 of the ToR. The Ordering Party does not specify a special way of fulfilling the condition by Contractors jointly applying for the order,

b) The Single Document referred to in point 8.2 The SIWZ is submitted by each of the Contractors jointly applying for orderse. These documents confirm fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion to the extent that each of the Contractors demonstrates fulfillment of the conditions for participation in the proceedings and no grounds for exclusion, c) Declaration on belonging to the same capital group, referred to in point . 8.3 The SIWZ is submitted by each of the Contractors,

d) They are obliged to the summonsOrdering Party, submit documents and statements referred to in point 8.7 of the Terms of Reference, whereby the documents and statements referred to:

-w. 8.7.1 The SIWZ shall submit respectively the Contractor / Contractor who / indicate the fulfillment of the condition in the scope and on the terms described in point. 6.2 of the SIWZ,

-documents and statements referred to in point 8.7.2 The ToR shall be submitted by each of the Contractors,

-documents about whichin point 8.7.3 The SIWZ shall submit accordingly to the Contractor / Contractor who / who confirms that the offered deliveries meet the requirements specified by the Employer.

- 6.2.2. economic or financial situation

The ordering party does not specify the condition in the abovementioned in the area.

Continued item III. contained in Section III.1.3): - - a) point 8.7.2 lit. a) The ToR shall submit information from the relevant register or, -in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence has the person concerned by the information or document, to the extent specified in art. 24 sec. 1 point 13), 14) and 21) of the Public Procurement Law, - - - b) point 8.7.2 lit. b) -d) The ToR shall submit a document or documents issuedin the country in which the Contractor has his registered office or place of residence, confirming that: - he is not in arrears with paying taxes, fees, social security or health insurance contributions or that he has entered into an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular, obtained the legally required exemption, postponement or distribution toinstallments of overdue payments or suspension in full of the implementation of the decision of the competent authority, - - its liquidation has not been opened or bankruptcy announced,

- 8.11. Documents referred to in point 8.10 lit. a) and lit. b) second indent of the SIWZ, should be issued not earlier than 6 months before the deadline for submission of tenders. The document referred to in point 8.10 lit. b) the first indent of the SIWZ, it should besubmitted no earlier than 3 months before the deadline,

- 8.12. If in the country in which the Contractor has its registered office or place of residence or place of residence has the person concerned, the documents referred to in point 8.10 of the ToR, it is replaced by a document containing the Contractor's declaration, indicating the person or persons authorized to represent himentation, or a statement of the person the document was to concern, submitted to a notary or before a judicial, administrative or professional self-government or economic authority competent for the seat or place of residence of the Contractor or the place of residence of that person. Provision of point 8.11 of the ToR apply, - - 8.14. Contractor having its registered office in the territory of the Republic of Poland, -in relation to a person residing outside the territory of the Republic of Poland, which is referred to in the document indicated in Chapter 8 of the Terms of Reference, submit the document referred to in point 8.10 lit. a) SIWZ, to the extent specified in art. 24 sec. 1 point 14 and 21 of the Public Procurement Law. If the country in which the person has the place where the document was to reside is located, no such documents are issued, it replacesit is a document containing a statement made by that person before a notary or before a judicial, administrative or professional self-regulatory or economic self-regulatory body. The provision of point 8.11 of the Terms of Reference of the first sentence applies, -12.1. The Contractor is obliged to pay a deposit in the amount of: -12.1.1. for part 1, orders: PLN 30, 000.00 (in wordse: thirty thousand zlotys),

-12.1.2. for lot 2, orders: PLN 5, 000.00 (in words: five thousand zlotys),

-13.4. The offer must be made in writing, under pain of nullity, except that EAT should be sent in an electronic form with a qualified electronic signature, - the Employer does not provide for the award of contracts referred to in Article. 67 sec. 1 point 6 and 7 of the P actzp,

- The two-month period specified in Section IV.2.6) should be understood as 60 days counted from the date set for submitting bids,

- Detailed information on the conditions of participation in the proceedings were included in Chapter 6, 7 and 8 of the ToR,
