The subject of the order is the provision of technical support services for products Oracle and ESRI with the delivery of new licenses, divided into 2 tasks:

See point II.1.4) -1. Contracts may be awarded for contractors who are not subject to exclusion from the proceedings and fulfill the conditions for participation in the proceedings. The contracting authority does not need to show grounds for no grounds for exclusion for subcontractors.

2. The ordering party, before giving the order, will call the contractor whose offer has been the highest rated, to submit in the claimat the date of filing the following declarations or documents:

2.1. confirming no grounds for exclusion from the proceedings:

a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids.

For Contractors who arecollective entities should also submit information regarding a collective entity,

b) a statement by the contractor about the lack of a final court decision or final administrative decision on default of taxes, fees or social security or health insurance premiums or in the event of such a decision or decision documents confirming the paymenth payments with possible interest or fines or conclusion of a binding agreement regarding repayment of such receivables,

c) declaration of the contractor about the absence of a ruling against him as a preventive measure of prohibition to apply for public orders

2.2 If the contractor has a registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point:

2.1 lit. a) - provides information from the relevant register, or in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the contractor has its registered office or place of residence is the person concerned by the information or document, in the scope specified in art 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law,

Documents, otions referred to in point 2.2 should be issued no earlier than 6 months before the deadline for submission of offers.

2.3 If the country in which the contractor has a registered office or place of residence has the person concerned, documents do not appear referred to in point 2.2. it is replaced by a document containing the contractor's statement, indicating the personor persons authorized to represent it, or a statement of the person the document was to concern, submitted to a notary or before a judicial, administrative or professional self-regulatory or economic body competent for the place of residence or place of residence of the person or place of residence. The provisions of point 7.4 regarding the dates of issuing documents, the pile- the Contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whom the document referred to in point 2.1. a), submits the document referred to in point 2.2, to the extent specified in art. 24 sec. 1 point 14 and 21 of the Public Procurement Law. If in the country where the person has a place of residence, whothe document was to concern, no such documents are issued, it is replaced by a document containing a statement of that person made before a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the place of residence of that person. The provisions of point 2.2 regarding the dates of issuing documents, apply accordingly.
