The subject of the order is comprehensive development, implementation and integration of the HIS and LIS systems together with the training module and delivery of computer devices with software within the framework of the project: "Informatisation and development of electronic services at the Neuropsychiatric Hospital. prof. Mieczysław Kaczyński SPZOZ in Lublin ", co-financed under the Regional Operational Program of the Lublin Province for the years 2014-2020.

The subject of the order is comprehensive development, implementation and integration of the HIS and LIS system together with the training module and delivery of computer devices with software within the framework of the project: "Informatisation and development of electronic services at the Neuropsychiatric Hospital. Professor Mieczysław Kaczyński SPZOZ in Lublin ", co-financed under the Regionalof the Operational Program of the Lublin Province for the years 2014-2020. A detailed description of the subject of the order constitutes Appendix No. 1 to the Terms of Reference - with reference to this condition, the Contracting Authority does not specify the minimum level of capacity - in relation to this condition the Contracting Authority does not set minimum levels of ability . In order to confirm that the contractor is not subject to exclusion pursuant to art. 24 sec. 1, art. 24 sec. 5 point 1 of the Act and meets the conditions for participation in the proceedings, referred to in art. 22 ust. 1b of the Act should be submitted:

- valid as of the day of submitting bids - a declaration submitted pursuant to art. 25 a para. 1 and 2 of the Act in the form of a single European order document, EAT according to the formula prepared in accordance with the standard form provided for in the RegulationCommission (EU) 2016/7 dated 5.1.2016 laying down a standard form for a single European order document (OJ L 31.3.16) constituting Appendix No. 4 to the ToR, in accordance with the following rules: - EAT should be sent in an electronic form bearing the qualified electronic signature. Statements of entities submitting the offer jointly and entities making availablepotentials submitted on the EAT form should be in the form of an electronic document, signed with a qualified electronic signature by each of them in the scope in which they confirm the circumstances referred to in art. 22 ust. 1 of the Act. Esp. Sec. VII SIWZ.

II. The ordering party will call the contractor whose offer has been the highest rated, to submit in the designated, not shorter than 10-days, valid dates as of the day of submitting the following statements or documents confirming no grounds for excluding:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for submission of tenders (in the form of the original or a copy certified as a true copy by the Contractor);

2) the contractor's statement that he has not been the subject of a final court judgment or final administrative decision on late payment of taxes, fees or social security or health insurance premiums or in the event of such a verdict or decision confirming the payment of such claims together with interest or interest; fines or binding bindingagreement on the repayment of these debts (in the original form);

3) the contractor's statement about the lack of a ruling against him as a preventive measure for applying for public orders (in the form of an original);

4) an excerpt from the relevant register or central register and information on business activities, if separate regulations require an entry in the register or register, for the purpose ofthe claim that there are no grounds for exclusion under art. 24 sec. 5 point 1 of the Act (in the form of an original or a copy certified as a true copy by the Contractor).

III. The ordering party will call the contractor whose offer has been rated the highest, to submit in the prescribed, not shorter than 10 days, the current date as of the day of submitting the following declarations or documents confirming the fulfillment of-terms of participation in the proceedings: - 2.1 List of services performed, according to the template constituting Annex No. 6 to the Terms of Reference (in the original form),

2.2 List of persons, according to the template constituting Annex No. 7 to the ToR (in the original form).

IV. The bid security has been defined in Chapter X SIWZ.

V. The Contracting Authority requires a performance bond to provide 10% of gross contractual remunerationon the offer (Chapter XVII SIWZ).

VI. Based on Article. 93 para. 1a of the Act, the Orderer may annul the procedure for awarding the contract, if the funds from the European Union budget, which the Employer intends to spend on completing the order, will not be granted.
