The subject of the orders includes the provision of services and robot in the field of summerego and winter maintenance of national roads managed by the General Directorate of National Roads and Motorways Branch in Lublin. District in Puławy. - Within the scope of the robot and services expected to be performed as part of the current (summer and winter) maintenance of national roads are the following assortments of the robot:

- ongoing maintenance roads,

- ongoing maintenance of engineering facilities,

- winter road maintenance

UtStructural neighbors are works that go beyond routine maintenance, the Ordering Party will commission them as part of individual orders.

1. The Contractor is obliged to attach to the offer a statement valid as of the day of submitting bids constituting preliminary confirmation that the Contractor:

a) is not subject to exclusion;

b) meets the conditions for participation in the procedure.

2. The statement referred to in item 1 above (in formie a single European order document prepared in accordance with the standard form provided for in the European Commission's executive regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU, hereinafter referred to as "single document" or "EAT"), the Contractor is obliged to send the Ordering Party in an electronic form with a qualified electronic signaturein accordance with the rules set out in IDW.

3. The Contractor who refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and compliance with them, in the scope in which he refers to their resources, the condition of participation in the proceedings, together with the offer, also submits uniform documents regarding these entities.

4. At the request of the Employer, to confirm the lack of subsTo be able to exclude the Contractor from participation in the proceedings, the Contractor is obliged to submit information, certificates, excerpt and statements referred to in - 5 points 1 to 6 and 9 and 10 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents which the contracting authority may request from the contractor in the proceedings for the award of the contract (Journal of Laws of 2016, item 1126).

5. If Contractorca. has its registered office or place of residence outside the territory of the Republic of Poland, and also if the person whose document relates is residing outside the territory of the Republic of Poland, the principles set out in - 7 and 8 above apply. Regulation.

6. Contractor, within 3 days from the date of posting on the website information referred to in art. 86 par. 5 at Pzp, submits to the Ordering Party a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. With the submission of a statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure. Voting is excluded from the procurement procedurea halo in relation to which any of the circumstances mentioned in art. 24 sec. 1 point 12 of the Public Procurement Law and art. 24 sec. 5 points 1, 2, 4 and 8 of the Public Procurement Law - 8. The ordering party, in accordance with art. 24 aa of the Public Procurement Law, provides for the possibility of firstly assessing the offers, and then examine whether the Contractor, whose tender was rated as the most advantageous, is not subject to exclusion-and meets the conditions for participation in the proceedings. - 9. The period of being bound by the offer referred to in point IV.2.6) of the announcement is 90 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires.

10. The contracting authority requires that the contract be carried out for a period of 72 months from 1 October 2018 (or from the date of signing the Agreement if the date is later). Performing Rothe bot and Services subject to the Agreement will take place on the day of the protocol of transfer of Roads to maintenance subject to the observance of intermediate deadlines specified for BUD, ZUD and BUM.

11. The Awarding Entity does not provide for the possibility of ordering, referred to in art. 67 sec. 1 point 6 of the Public Procurement Law

12. The Employer reserves the obligation of the Contractor to perform personally the key parts of the order, iethe on-site contractor will perform the Robots and Services in the scope of: - - road surface repairs, - winter road maintenance (ZUD).

Which the Ordering Party considers key elements of the Contract and which can not be subcontracted.
