Task 4 "Construction of sanitary sewage system Czarnochowice Gmina Wieliczka. under the Project entitled "Construction of sewage system in the Wieliczka Commune stage IV"

Reference Number: WZP.271.23.2018

The order includes the construction of a network and sanitary sewer extractor in the town of Czarnochowice with diameters DN200 and the implementation and commissioning of an unmanned pumping station, including:

Main Site: Czarnochowice, gmina Wieliczka.

1. The order includes the construction of a network and reach for sanitary sewer in the town of Czarnochowice with diameters DN200, from solid homogeneous pipes and-PVC-U cup-shaped fittings with elongated socket joined to rubber gaskets or pipe connectors for the following requirements: - PVC-U material solid, homogeneous, hardened un-softened,

- highest tightness of joints and chemical resistance,

- possession of approvals for the whole range of pipes and fittings,

- admission for use in the area of mining damage and III Category

2. For sewage sectionssanitation realized in the technology of controlled drilling and slitting, use: - glazed stoneware pipes in accordance with PN-EN 295 connected to a molybdenum steel muff with a rubber-elastic seal, strength ≥ 80 KN / m,

- steel pipes SP-CZ-B1 as jacking protective pipes terminated with rubber sleeves PVC pipes SN8 as proper pipes, pulled into jacking pipesdistance spacers for drilling applications requiring the use of protective pipes.

3. Sanitary sewer routes should be made in accordance with: - existing or anticipated buildings, - - roads, ditches, fences, - existing sewage system, - - water supply network, - - power and telecommunications equipment, - - arrangements and conditions with property owners .

4. The project accepted a minimal admissiontotal reduction of ducts at the level of 0.5% for DN 200 channels. Collectors are designed to be made in trenches with vertical walls in 85% mechanically and 15% manually, with surface drainage, drainage or wellheads.

5. The order also applies to the implementation and commissioning of a maintenance-free sewage pumping station, which will work in an automatic system, and the operation of the pumps will be controlled by an electronic systemcooperating with the sewage level sensor. The transmission of signals about work and emergency conditions will take place via a GSM modem. the pumping station, as a whole, should have a declaration of performance in accordance with PN-EN 12050-1: 2002 and be CE marked.

The pumping station will be powered from the measuring connection. The pumping station provides for a ride terminated by a maneuvering yard to theRacani. The area of the pumping station will be fenced, equipped with an entrance gate, pavement with paving stones and a lighting lamp.

7. The detailed scope of the orders is specified in the following documents included in the project documentation:

- Construction and executive design,

- Technical Specifications for Construction Work and Acceptance,

- Bills of quantities - robot's work should be treated as a supplementary document, and the lack of any item in the pre-measure will not release the Contractor-from the necessity of its implementation, if it results from a construction or execution project,

- Inventory of greenery,

- Geotechnical documentation.

1. Contractors who are not subject to exclusion pursuant to art. 24, item 1, points 12-23 of the Code of Association - 2. By the end of the deadline for submission of tenders, each contractor must submit the current, as of the day of submitting bids, to the extent indicated byfrom the Employer

2.1) In the case of joint application for contractors, JEDZ submits each of the contractors jointly applying for the order.

2.2) Contractor who intends to entrust the execution of a part of the contract to subcontractors not being the entities referred to in Article. 22a uPzp, is obliged to include in the EAPO appropriate information about it.

2.3) Contractor who-refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and compliance with them, in so far as it relies on their resources, the condition of participation in the proceedings also consists of EED regarding these entities

3. On request in accordance with art. 26 par. 1 uPzp The Contractor is obliged to submit:

3.1) Information from the National Criminal Register in the scope specified in-Art. 24 sec. 1 pt. 13, 14 and 21 uPzp issued not earlier than 6 months before the deadline for submitting bids;

3.2) Contractor's statements about the lack of issuing a final court judgment or final administrative decision on default of taxes, fees or social security contributions or or in the event of such a decision or decision of documents-confirming the payment of these receivables together with any interest or fines or entering into a binding agreement regarding the repayment of such receivables.

3.3) Contractor's statements about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders.

4. Each of the Contractors jointly applying for the award of the contract is obliged to submitat the request of the contracting party referred to in point 3.1) 3.3). Contractor who relies on the ability or situation of other entities on the terms set out in Article 22a uPzp is obliged to present on the ordering party's request in relation to these entities the documents listed in point 3.1) 3.3).

6. If the contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point 3.1), it shall submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which the contractor has its registered office or place of residence concerns information or document, to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law issued not earlier than 6 months before the deadline for submission of tenders. If the country in which the contractor has its registered office or place of residence or place of residence has a person to whom the document refers, the documents referred to in point 3.1 shall not be issued. it is replaced by a document containing the contractor's statement, indicating the person or persons entitled to itabout his representation, or statement of the person to whom the document was to be applied, submitted to a notary or before a judicial, administrative or professional self-government or economic authority competent for the seat or place of residence of the contractor or place of residence of that person - issued not earlier than 6 months before expiry of the deadline for submission of tenders.

8. Contractor in theserminie 3 days from the date of posting on the website information referred to in art. 86 par. 3 uPzp, will provide the orderer with a declaration of belonging or non-affiliation to the same capital group, referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. Along with the submission of the statement, the contractor will provide evidence that links with another contractor do not lead to distortion of competitionin the procedure for ordering. In the event of a joint application for the award of each of these Contractors.

The Employer does not set a condition in this respect.

1. The Contractor, under pain of rejection, offers a bid bond: in the amount of PLN 15, 000.00 (five hundred thousand zlotys 00/100) at the latest until 28.8.2018 at 09: 50;

2. The orderer prescribes the use of the circle procedureeated in art. 24aa uPzp.

3. The Contracting Authority does not provide for the conclusion of a framework contract.

4. The ordering party does not anticipate a dynamic purchasing system

5. The Contracting Party does not plan to conduct an electronic auction.

6. The Contractor is obliged to submit together with the offer the following documents:

6.1) The offer form completed and signed The offer form based on the template form constitutesAnnex 1 to the ToR. In the case of a multipage document, it should be signed on every page of it.

6.2) Obligation of the entity to place the necessary resources for the needs of the order or other document if applicable. In order to assess whether the contractor, relying on the abilities or situation of other entities under the principles set out in Article 22a uPzp (point 5.4 of the SIWZ), bday had the necessary resources to the extent that allows proper implementation of the public order and assess whether the relationship between the contractor and these entities guarantees real access to their resources, the contracting authority requests the submission of documents along with the offer (in particular, a written commitment) which specify in particular: - the range available Resource contractors of another entity,

7. Offers should be submitted in writing.

9. The order is co-financed from the Operational Program Infrastructure and Environment 2014-2020 action 2.3 "Water and wastewater management in agglomerations "Priority axis II" Environmental protection, including adaptation to climate change ".

10. Due to the fact that the value of the construction works in relation to the entire Project exceeds the amounts referred to in art. 11 para. 8 of the Public Procurement Law, the provisions applicable to contracts with values exceeding the amounts referred to in art. 11 para. 8 uPzp.
