1. The subject of the order ist: Supply of medical equipment and furniture to the Medical Simulation Department of the University of Medical Sciences at ul. Chałubińskiego 7a in Wrocław. The subject of the orders was divided into 9 (nine) parts separately assessed.

The subject of the Part A order is delivery:

1. Office chairs - 42 items,

2. Travel tablesfolding table top - 12 pcs.

1. Contractors, who are not subject to exclusion, may apply for the award of the order.

2. The Awarding Entity does not set the conditions for participation in the procedure. - In the case of Contractors jointly applying for the award of a contract, the condition referred to in point 1, is met if none of the entities submitting a joint bid is subject to exclusion.

3. Ordering prit will see the Contractor's exclusion also based on art. 24 sec. 5 points 1 and 8 Pzp (optional grounds for exclusion of the Contractor), ie: -1) in relation to which the liquidation has been opened, in the restructuring arrangement approved by the court, the creditors are expected to settle by liquidation of their property or the court ordered the liquidation of its assets in the mode of art. 332 par. 1 of the Act of don 15.5.2015. Restructuring law (consolidated text - Journal of Laws of 2019 item 243, as amended) or whose bankruptcy was announced, with the exception of the Contractor who, after the bankruptcy, concluded an arrangement approved by a valid court order if the arrangement does not provide for the satisfaction of the creditors by liquidation of the bankrupt's assets, unless the court ordered the liquidation of its assets pursuant to art. 366 para.1 of the Act of 28.2.2003 Bankruptcy Law (consolidated text - Journal of Laws of 2019 item 498),

2) which violated obligations regarding tax payments, fees or social security or health insurance premiums, which the Employer is able to demonstrate by means of appropriate evidence, except in the case referred to in Article 24 sec. 1 point 15 Pzp, unless the Contractor has made payments duetaxes, fees or social security or health insurance contributions, including interest or fines, or have entered into a binding agreement on the repayment of these claims. The contractors together with the offer should submit a statement on the day of submitting bids regarding non-subject to exclusion. The information contained in the statement will constitute preliminary confirmation that the Contractor is not subject to exclusion.The Contractor submits this declaration in the form of a single document. In the event of a joint application by the Contractors, a single document shall be submitted by each of the Contractors jointly applying for the order. These documents confirm the lack of grounds for exclusion to the extent that each of the Contractors shows no grounds for exclusion. Contractor who intends to entrust the execution of a part of the orderto subcontractors, in order to demonstrate the absence of grounds for exclusion from participation in the procedure, submit uniform documents regarding subcontractors. The contractor, who relies on the resources of other entities, in order to demonstrate the absence of grounds for exclusion against them, also submits uniform documents concerning these entities. The ordering party before ordering will call Wykothe creator whose offer has been rated the most, to submit in the prescribed, not shorter than 10 days, the current date as of the day of submitting the following statements or documents:

1) Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the PPL, issued not earlier than 6 months before the deadline for submitting bids;

2) Certificates of the competent directora tax office confirming that the Contractor is not in arrears with payment of taxes issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, of particularity obtained the legally required exemption, postponing or spreading overdue payments into installments or suspending in full the decision of the competent authority; - 3) Certificates of the local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions exhibited ate earlier than 3 months before the deadline for submission of bids or other document confirming that the Contractor has made an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or installment of overdue installments or suspension in the entire implementation of the decision about therganu;

4) A copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion pursuant to art. 24 sec. 5 pt 1 Pzp (if the Contractor is an entity registered in Poland, the Ordering Party will independently download the document using free and general accessh) 5) Declaration of the Contractor about the lack of issuing to him a final judgment of the court or final administrative decision on defaulting taxes, fees or social security or health insurance premiums or - in the event of issuing such a decision or decision - documents confirming the payment these receivables together with any interest or fines or fora declaration of the Contractor about the repayment of these receivables;

6) Contractor's statements about the lack of a ruling against him as a precautionary measure prohibiting to apply for public orders;

7) Contractor's statements about not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (consolidated text - Journal of Laws of 2018, item 1445, withLate. d.).

6. The Employer demands from the Contractor, which relies on the abilities or situation of other entities on the terms specified in art. 22a Pzp, presenting in relation to these entities the documents listed in item 5.1 5.7.

7. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to in sub-item 5.1-5.4:

1) regarding item 5.1 provides information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor is established or resides in the person concerned by the information or document, in the scope specified in art 24 sec. 1 item 13, 14 and 21 PzP.

2) regarding items 5.2-5.4 consists of dthe document or documents issued in the country in which the Contractor has his registered office or place of residence, confirming that: a) he is not in arrears with taxes, fees, social security or health insurance contributions or that he has entered into an agreement with the competent authority regarding the repayment of receivables together with any interest or fines, in particular obtained the legally required exemption, postponing or spreading overdue payments into installments or suspending in full the decision of the competent authority, - b) its liquidation has not been opened or bankruptcy has been declared. Documents referred to in points 7.1 and 7.2 lit. b, should be issued not earlier than 6 months before the deadline for submission of tenders, and the document referred to in sub-item 7.2 lit. a, should be exhibited not inlater than 3 months before the deadline for submitting offers.

1. The Contracting Authority does not provide for the possibility of awarding contracts referred to in Article 67 sec. 1 point 7 PzP.

2. The Contracting Authority does not provide for the conclusion of a framework agreement.

3. The ordering party requires a bid bond in the amount of: - For part A, orders: PLN 300.00 (in words: three hundred zlotys and PLN 00/100),

For part B, orders: PLN 11, 600.00 (in words: oneaś thousand, six hundred zlotys and 00/100),

For part C orders: PLN 150.00 (in words: one hundred and fifty zlotys and PLN 00/100),

For part D orders: PLN 1, 450.00 (in words: one thousand four hundred and fifty PLN and 00/100),

For part E, orders: PLN 7 400.00 (in words: seven thousand four hundred zlotys and 00/100),

For part F orders: PLN 2, 000.00 (in words: two thousand zlotys and 00/ 100). For parts G orders: PLN 1, 000.00 (in words: one thousand zlotys and 00/100),

For part H orders: PLN 900.00 (in words: nine hundred zlotys and 00/100),

For parts and orders: PLN 1, 550.00 (in words: one thousand five hundred fifty zlotys and 00/100).

4. The bid bond can be contributed in one or several of the following forms:

1) money;

2) bank guarantees or suretiesa savings and credit union, however, the surety is always a cash guarantee;

3) bank guarantees;

4) insurance guarantees;

5) sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (consolidated text - Journal of Laws of 2018, item 110, as amended).

5. Awardingrequesting security only for part B (Advanced ALS simulators with trauma module). In the scope of other parts of the order, the Ordering Party does not request security.

6. The amount of the security is 5% of the total price specified in the offer.

7. The security for due performance of the contract should be paid in PLN.

8. Security may be provided as per the choiceContractors in one or several of the following forms:

1) money,

2) bank guarantees or social-savings credit sureties, except that the obligation of the cash register is always a cash liability;

3) bank guarantees;

4) insurance guarantees;

5) sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of November 9, 2000 about the creation ofeniu of the Polish Agency for Enterprise Development.

9. The Contractor remains bound by the submitted offer for a period of 60 days

10. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires
