CPV Description: Services provided by medical laboratories will be used in order to ensure the medical act for the patients of the County Emergency Hospital " St. Pantelimon "Focşani.

Main Site: The headquarters of the St. Panteli County Emergency HospitalThe acquisition implies the conclusion of a service contract in which the provider will oblige to provide the medical services (laboratory analysis) within the agreed period and according to the obligations assumed by the contract. The provider will also be obliged to work in the space provided by the purchaser, for which he will pay a monthly rent according to the notebook-Information and formalities necessary for evaluating if requirements are met:

Proposed economic operators / associate / third party / proposed subcontractors must not be present in the situations referred to in art. 164, 165, 167 of Law 98/2016. The way in which the fulfillment of the requirement can be demonstrated: the DUAE will be completed by the participating economic operators-to the award procedure, with information related to their situation. Presentation of the criminal record of the economic operator and of the members of the administrative, management or supervisory body of the economic operator or of the persons having the power of representation, decision or control within the economic operator as it results from the certifying certificate issued by ONRC / the constitutive act. The supporting documents proving the fulfillment of the assumed by completing the DUAE shall be submitted, only at the request of the contracting authority, only by the tenderer ranked first in the intermediate rankings drawn up after applying the award criterion, only on-line. These documents can be: fiscal attestation certificates, certificates of absence of debts(local budget, state budget, etc.) at the moment of their submission, or any document considered as edifying in this respect in the country of origin or in the country where the tenderer is established, such as certificates, criminal records or other equivalent documents issued by competent authorities in the countryespectiva. Requirement 1: People-Romanian legal / physical: Certificate issued by the National Trade Register Office, from which the object of activity of the respective economic operator is derived. The subject of the contract must have a correspondent in the CAEN code of the certifying certificate issued by the ONRC. The information contained in this document will be actual / current at the time of submission. Pantelimon "Focsani, presented in the offer ISO Certification for the provision of laboratory analyzes for the provision of clinical studies The way to prove the fulfillment of the requirement: it will be completed by the economic operators participating in the award procedure with the information related to their situation. The supporting documentswhich proves the fulfillment of the assumed by completing the DUAE, shall be presented, at the request of the contracting authority, only by the tenderer ranked in the first rank in the interim ranking prepared after applying the award criterion. years, 2015, 2016, 2017, through which the bidder can doveeconomic and financial capacity (eg the balance sheet targeted and registered with the competent bodies or, as the case may be, the annual reports, creditworthiness letters from banks or recognized financial audit firms and accountants).
