School catering services.

Provision of catering service in primary schools in the municipality of Tarnowo Podgorne

Reference Number: WOP.271.45.2018

The object of the order is to provide catering service consisting in the preparation, delivery of dinners and the release of meals and table service in primary schools in the municipality of Tarnowo Podgorne. 2. Conditions to implementji required for all sites in all parts:

Main Site: Primary School No. 1 in Tarn-w Podgorny.

1. The scope of service provision in individual establishments includes:

Part I:

Primary School No. 1 John Paul II in Tarnow Podgorny, ul. Szkolna 5, 62-080 Tarnowo Podgorne:

- preparing, delivering and issuing meals for schoolchildren in andresulting from the status of orders.

Average number of lunches - 5, 200 per month. The number shown may be higher or lower in individual settlement months and will depend on the current demand.

- meals are issued in three rounds from 12:30 to 14:00,

- meals and drinks will be issued on vessels made of durable plastic artificial allowfor safe washing in dishwashers and metal cutlery, which the contractor provides on its own,

- it is excluded to use dishes and disposable cutlery,

- soup served in vases, which the contractor provides on its own.

3. Meals must meet the following quality conditions:

1) a two-week menu should be varied; the type of soup can not be repeated in a week, andsecond dish in a decade (10 days);

2) in a two-week menu in one week 4 times dinner with the second meat dish (beef, pork, pork, pork - portioned), and in the remaining one day: fish dish; in the following week, a lunch with the second meat dish is allowed 3 times a week (beef, pork, pork, portions of pork), and in the remaining two days a vegetarian dish, chilledor fish;

3) at least once every two weeks, the second dish with sea fish (cod, pollock, hake, salmon); the ordering party allows only the fillet only;

4) in the menu should prevail cooked, baked and stewed dishes, less fried; the meat must be soft;

5. The Contractor will prepare meals from his own products. Meals must be prepared in accordance with the rules set out in-the Act on Food Safety and Nutrition (vol. Journal of Laws of 2017, item 149, as amended) and the Regulation of the Minister of Health of 26.7.2016 on groups of foodstuffs intended for sale to children and youth in units of the education system and the requirements that must be met by foodstuffs used in the context of mass catering for children and adolescents in these units (OJ of 2016th, item 1154) -6. Meals must meet dietary norms for the appropriate age group (weight and caloric content suitable for adolescents from 6 to 16 years of age) and prepared under the supervision of the dietitian of the Contractor. The contractor will prepare and deliver meals while maintaining sanitary and epidemiological requirements in terms of personnel and production conditions, and will take responsibility for theirzygotowanie in accordance with applicable norms and laws.

7. The contracting authority requires that the persons implementing the subject of the orders that will perform the activities actually related to the subject of the order described in the specification of relevant conditions of the contract were employed on the basis of a contract of employment, 1 full-time job. * Number of people employed for lot 1: minimum 3 axis- the Ordering Party does not set any requirements in this respect.

1. The ordering party conducts proceedings using the reverse procedure in accordance with art. 24 aa of Public Procurement Law

2. The Contractor must, prior to the deadline for submission of tenders, submit a bid bond with the following value: - Part 1 - PLN 10, 000.00 (in words: ten thousand zlotys).

Part 2 PLN 12, 000.00 (in words:-twelve thousand zlotys).

The ordering party does not require a performance bond to be provided.

1. The offer must include the following statements and documents:

1) a completed offer form prepared using the template constituting Annexe No. 1 to the ToR, including in particular: the total gross offer price, the obligation regarding the deadline for order fulfillment, warranty periodand the terms of payment, a statement of the period of being bound by the offer and acceptance of all provisions of the ToR and the model contract without reservations, and information about which part of the contractor the Contractor intends to entrust to the subcontractor; - 2) statements in the form of EAT listed in Chapter VIII point 1-3 of these ToR, submitted in accordance with the requirements contained in chapter x of the ToR,

3) obligation of the third party-(in the original) to make available to the Contractor the necessary resources for the order (if the contractor invokes the third party resources);

4) a separate list of services (along with a copy of evidence confirmed for compliance with the original) related to the preparation, delivery and issuing meals provided continuously for a minimum of 10 months worth at least 100, 000, PLN 0.00 each, submitted for the assessment of offers in the professional experience criterion.

2. If the offer is signed and the document is certified as true copies by a person not mentioned in the registration (registration) document of the Contractor, the offer must be accompanied by an appropriate power of attorney in the original or a notarized copy.

3. The contractor can meet the requirements for confirmationconditions referred to in Chapter VII, point 1 point 2 of the ToR in appropriate situations and in relation to the order in question, rely on the technical or professional abilities of other entities, regardless of the legal nature of the legal relationship that it has with it.

4. Contractor within 3 days from the date of posting on the website information referred to in art. 86 par.5 of the Public Procurement Law, will provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 Public Procurement Law. Along with the submission of the statement, the contractor may provide evidence that links with other contractors do not lead to distortion of competition in the procurement procedure. The pattern of the statement is attached as Annex 5 to SIGM.
