1. The subject of the order is the delivery, divided into parts (24 parts) of equipment and software for digitization for the partners of the Culture 2.0 project within the scope of the project "Culture in range 2.0", implemented as part of the Regional Operational Program of the Kuyavian-Pomeranian Voivodeship for-2014-2020, Priority axis 2. Digital region, Measure 2.2. Digital availability and usefulness of public sector information and resources of science, culture and regional heritage

1. The subject of the order is the delivery, broken down into parts (24 parts) of equipment and software for digitalization for the partners of the Kultura project within 2.0 "within the framework of the" Culture in range 2.0 "project, -implemented as part of the Regional Operational Program of the Kujawsko-Pomorskie Voivodeship for the years 2014-2020, Priority axis 2. Digital region, Measure 2.2. Digital accessibility and usefulness of public sector information and resources of science, culture and regional heritage

2. A detailed description of the subject of the order is provided in Annex 8 to the SETC

1. Vetch may apply for the orderthe ones who are:

1.1. are not subject to exclusion pursuant to art. 24 sec. 1 of the PZP Act and art. 24 sec. 5 point 1 of the Public Procurement Law - List of statements and documents confirming the fulfillment of the conditions for participation in the proceedings and no grounds for exclusion: - The contractor appends to the offer or application for participation in the procedure - 1.1. Current on the day of submitting offers a statement in the form of a uniform dthe document on the EAT form (Annex 1 to the ToR) in an electronic form with a qualified electronic signature. *

1.2. In the case of joint application for contractors, the statement referred to in item 1.1 of the SIWZ is submitted by each of the contractors jointly applying for the order. This statement confirms the fulfillment of the conditions for participation in the proceedings and the lack of grounds inexclusion in the scope in which each of the contractors shows compliance with the conditions for participation in the proceedings and no grounds for exclusion.

1.3. The Contractor who refers to the resources of other entities attaches the obligation of other entities to the Contractor to dispose of the necessary resources for the needs of the order, if applicable (the obligation of these entities should be submitted in the original).

1.4. The Contractor who refers to the resources of other entities in order to demonstrate the absence of the existence of a foreclosure against them and fulfillment - in so far as it relies on their resources - the conditions for participation in the submission are also made by the statement referred to in Chapter. V point 1 item 1.1 SIWZ (form EAT), regarding these entities

Stage II.

2. Contractor, within 3 days of placing on the siteI internet information referred to in Article 86 of the 5 (information from the opening session of the offers), provides the orderer with a statement on belonging to or a lack of affiliation to the same capital group, referred to in art. 24 sec. 1 point 23 of the PPL Act, ie: (Attachment 3do SIWZ). - Stage III (submission of documents at the request of the ordering party).

3.1 no grounds for exclusion:

3.1.1 information from Krajowy Rejecriminality to the extent specified in art. 24 sec. 1 point 13, 14 and 21statement, issued not earlier than 6 months before the deadline for submitting bids,

3.1.2 extract from the relevant register or from central records and business information, if separate provisions require entry in the register or records, in order to confirm lack of a base excused from Article 24 sec. 5 point 1Public Procurement Law Acts

3.1.3 the contractor's statement that he has not issued a final court judgment or final administrative decision about defaulting taxes, fees or social insurance or health insurance premiums, or - in the event of such a verdict or decision - documents confirming payment of such receivables with interest or interest; fines or limitationbinding contract on the repayment of these debts (Annex 4);

3.1.4 the contractor's statement about the lack of a ruling against him as a preventive measure forbidding to apply for public orders (Annex 5);

If the contractor relies on the ability or situation of other entities under the terms of art. 22a Act of PPL, presents in relation to these entities the documents listed underkt 3.1.1 4.

Note:

If the contractor has a registered office or place of residence outside the territory of the Republic of Poland, the Contractor is required to provide documents specified in the ToR.

The ordering party does not place a specific condition in this respect - the Contractor is obliged to pay a bid bond

2. The contracting authority requests a performance guarantee of 5% of the remunerationand the due Contractor for the entire period for which the contract will be concluded (gross).

3. The Contractor shall attach to the offer, in addition to the documents referred to in Section III of this notice: Completed bid form constituting Annex 2 to the Terms of Reference, including, in particular, the offer price and a list of subcontracts
