The subject of the order is the provision of agency services for the sale of railway tickets on behalf of and for the benefit of the Employer in places of agency service sale of tickets at stations: Radomsko, Rogow, hereinafter referred to as "Stations", in premises provided byThe contractor. The subject of the order is the provision of agency services for the sale of railway tickets at the stations: Radomsko, Rogow.

1. Contractors who meet the conditions for participation in the Procedure may apply for the award of the contract and are not subject to exclusion from the Proceedings. The Contracting Authority excludes the Contractor from the proceedings in cases specified in art. 24 sec. 1 item 12-23 and para. 5 point 1 (from claimof Article 133 ust. 4) of the Act of 29.1.2004 - Public Procurement Law (Journal of Laws of 2018, No. 1986, as amended), hereinafter referred to as the "PPL Act".

2. To confirm the absence of grounds for exclusion of the Contractor from the Proceedings, referred to in paragraph 1, and in order to confirm compliance with the conditions for participation in the Procedure, referred to in Section III.1.3, the Contractor must submit within the deadline for submittingert valid as at the day of submitting bids a statement regarding the lack of grounds for exclusion and fulfillment of the conditions for participation in the proceedings. The information contained in the statement constitutes preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the Proceedings. The Contractor shall submit the statement referred to in the preceding sentences in the form of a Single European DocumentOrder (hereinafter referred to as "EAT"), in the form of an electronic document with a qualified electronic signature, the Contractor may limit himself to completing the section α in part IV EAT and does not have to fill in any of the other sections in Part IV. Section D EAT information whether it intends to entrust the execution of some orders to subcontractors andproviding subcontractors. Additionally, the Ordering Party requires the Contractor to indicate in PART IV Section C item 10 EAT parts of the orders, which he intends to entrust to the subcontractors.

3. The contractor whose tender will be selected as the most advantageous will be requested to submit the following documents:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids in the Proceeding;

2) Contractor's statement that he has not issued a final court judgment or final administrative decision on late payment of taxes, fees or social security contributions or or in the event of such a decision or decision being issued, the documents are confirmederasing the payment of such receivables with possible interest or fines or conclusion of a binding agreement regarding the repayment of such receivables; - 3) declaration of the Contractor about the lack of a ruling against him as a preventive measure of the prohibition to apply for public orders; - 4) extract from the relevant register or Central Recording and Information on Economic Activity, if odrdeep rules require entry in the register or in the records, in order to confirm the absence of grounds for exclusion under art. 24 sec. 5 point 1 of the Public Procurement Law

4. Contractor having its registered office or place of residence outside the territory of the Republic of Poland and the Contractor having its registered office in the territory of the Republic of Poland, but the person concerned by the document resides outside the territory of Rzecfrom Poland, the documents listed in para. 3 points 1 and 4 in accordance with - 7 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the procedure for awarding the contract (Journal of Laws of 2016, item 1126 with .).

1. The Contractor is obliged to pay a bid bond in the amount of PLN 5, 500.00.

2. The bid bond must be submitted before the lapsethe deadline for submitting offers in one or several forms specified in art. 45 par. 6 Public Procurement Laws
