The subject of the order is the reconstruction of the roof with insulation in the Primary School No. 36 ul. Czołgistow.

1. Object fromit is possible to reconstruct the roof with insulation in the Primary School No. 36, ul. CzoĹ, gistow.

2. The subject of the orders includes:

a) performance of comprehensive project documentation including construction design, detailed design, Technical Specification of Construction Work and Acceptance, and robot,

b) construction work based on project documentation;

c) supervision-copyright.

For the first stage of the reconstruction of the roof for the investment task titled "Reconstruction of the roof with insulation in the Primary School No. 36 ul. Tankers.

3. A detailed description of the subject of the order has been included in Annex 3 to the ToR and in the functional and application program constituting Annex 3a to the ToR.

4. The minimum required warranty period is 36 months.

1. Ordering party before grantingorder, will call the Contractor, whose offer was the highest rated to submit in the designated, not shorter than 10 days, valid as of the day of submitting statements or documents confirming the circumstances referred to in Article. 25 ust. 1 of the Public Procurement Law. The Ordering Party will call the Contractor, in particular to submit the following documents:

1.1. information from the National Registryru prison in the scope specified in art. 24 sec. 1 points 13, 14, and 21 of the Act on Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,

1.2. certificate of the competent head of the tax office confirming that the contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submission of bids, or otherthe document confirming that the contractor concluded an agreement with the competent tax authority regarding the repayment of such receivables together with any interest or fines, in particular it obtained the legally required exemption, deferral or installment payment of overdue payments or suspension of execution of the decision of the competent authority in its entirety

1.3. certificate of the appropriate local organizational unitSocial Security Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the contractor has concluded an agreement with the competent authority on repaymentsh receivables with possible interest or fines, in particular obtained legally required exemption, postponement or spreading overdue payments into installments or suspension of the execution of the decision of the competent authority in full,

1.4. the contractor's statements about not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (i.e.z.U. from 2017, item 1785, as amended);

1.5. excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Public Procurement Law,

1.6. the contractor's statement that the court did not issue a final judgment against himu or a final administrative decision on default of taxes, fees or social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming payment of such claims together with any interest or fines or conclusion of a binding agreement regarding repayment of such claims; 1.7. statements by the contractor o-lack of a ruling against him as a preventive measure of the prohibition to apply for public orders.

In reference to the conditions for participation in proceedings regarding financial or economic situation, the Employer requires that the Contractor has adequate liability insurance in respect of the business related to the subject of the order for a guarantee sum no methan PLN 200, 000.00.

1. The Contractor is obliged to submit a completed offer form prepared according to the template constituting Annex No. 1 to the ToR.

2. The Contractor is obliged to attach to the offer:

2.1. valid as of the date of submitting bids constituting preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings in the form of a uniform Europeanego of the order document, drawn up in accordance with the specimen standard form specified in the European Commission's implementing regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU, hereinafter referred to as "Single Document." The Contractor is obliged to submit a Single Document to the extent specified in Annex 4 to the Terms of Reference, subject to the provisions of clause 5 in Chapter XV of the Terms of Reference.and is obliged to submit a Single Document in an electronic form with a qualified electronic signature by means of electronic communication, ie the Platform for electronic processing of public Procurement orders available at https://umsosnowiec.logintrade.net/.

2.2. documents referred to in Chapter VIII, point 2) and 4) of the ToR,

2.3. appropriate original or notarycertifiable power of attorney, if the offer is signed by a proxy or in the case when the authorization to sign the offer does not result from the Contractor's registration document.

3. In order to assess whether the Contractor, relying on the abilities or situation of other entities under the principles set out in Article 22a of the Public Procurement Law, will have the necessary resources to the extent possiblewhich correctly drafts the public order and assesses whether the relationship between the contractor and these entities guarantees effective access to their resources, the Ordering Party requests documents, which specify in particular:

3.1. the scope of resources of another entity available to the Contractor;

3.2. the manner of using the resources of another entity, by the Contractor, while executing the order;

3.3. scope and period of participationof another entity when executing a public order;

3.4. whether the entity, on the capacities of which the Contractor relies in relation to the conditions of participation in the proceedings concerning education, professional qualifications or experience, will carry out construction works whose indicated capacities relate to - 4. Contractor, within 3 days of posting on the website the information referred to-in art 86 par. 5, shall provide the Ordering Party with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Act on Public Procurement Law in accordance with the model constituting Annex 2 to the Terms of Reference. In the case of being part of the same capital group, the Contractor may submit together with the declaration documents or information confirming thate links with another contractor do not lead to distortion of competition in the procurement procedure. The Contractor will be bound by the submitted offer for a period of 60 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires.

6. The Contractor is obliged to submit a bid bond before the deadline for submitting bids in the amount of PLN 4, 000.00 (in words: PLN 4, 000)00/100). -7. Before signing the contract, the Contractor is required to provide a performance bond for the amount of 10% of the offer price given in the Offer Form. Contractors and other persons, if their interest in obtaining the contract suffered or may be damaged as a result of violation of the provisions of the Public Procurement Law, are entitled to legal protection measures described in Section VI of the Act-public orders.
