The subject of the service is full sequencing-exomic (analysis of the whole-exome sequencing WES genome coding part) along with the preparation of libraries for sent samples.
Genetic material sent to the Contractor will be human genomic DNA isolated from peripheral blood by salting out.
The Contractor will perform full-exome sequencing and send them results along with bioinformatic analysis within 45 days from the date of receipt of prcrib.

Contractor's statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security contributionstne, or in the event of such a judgment or decision - documents confirming the payment of these debts together with any interest or fines or the conclusion of a binding agreement on the repayment of these debts;
3. the Contractor's statement of the lack of a decision against him as a precautionary measure forbidding public procurement;
4. an excerpt from an appropriate registeror from the central register and information on economic activity, if separate provisions require an entry in the register or the register, to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the Act;

2. a certificate from the appropriate organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social or health insurance contributions, issued not earlier than 3 months before the deadline for submitting offers, orb another document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferral or installment of overdue payments provided for by law or withholding in full the decision of the competent authority;
3. Contractor's declaration of non-payment of taxesand local fees, referred to in the Act of 12.1.1991 on local taxes and fees (i.e. from 6.7.2018 Journal of Laws of 2018, item 1445, as amended);

II. The awarding entity envisages using the procedure specified in the disposition of art. 24aa Pzp ('proceinverted dura ").
III. The bid must be secured by a bid bond of PLN 7, 000.00. The bid submission deadline is the same as the bid submission deadline.
IV. The contracting authority does not provide supplementary orders referred to in Article 67 para. 1 point 6 and 7 of the Public Procurement Law
V. Documents and statements from this point are submitted at the request of the Employer pursuant to Article 26 (1) of the Public Procurement Law:
- the Contractor forharnessing the ability to perform WES (whole exome sequencing) analysis using genomic DNA submits at least three anonymized results of such analyzes from the last 36 months before the deadline for submission of tenders, and if the period of business is shorter in that period .
VI. If the Contractor has its registered office or place of residence outsidethe territory of the Republic of Poland, and in the country in which the Contractor has its registered office or place of residence or place of residence the person to whom the document relates does not seem the documents referred to in the announcement, the provisions of the Regulation of the Minister of Development of 26.7.2016 on on the types of documents that the contracting authority may request from a contractor in a procurement procedureof prisoners (Journal of Laws 2016 item 1126).
VII. The Contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whom the document referred to in the announcement relates, submits the document in the scope specified in art. 24 paragraph 1 points 14 and 21 of the PPL applying the provisions of the Regulation of the Minister of Development of 26.7.2016 on the typeo documents that the contracting authority may request from the contractor in the procurement procedure (Journal of Laws 2016 item 1126).
