1. certificate of the competent head of the tax office confirming that the Contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor froman agreement with the competent tax authority regarding the repayment of such receivables, together with any interest or fines, in particular obtained the legally required exemption, deferral or payment in installments of overdue payments or suspension in full of the implementation of the decision of the competent authority; certificate of the relevant organizational unit of the Social Insurance Institution or Kas. Agricultural Social Insurance or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has agreed with the competent authority regarding repayment of these receivables with possible interestand / or fines, in particular he obtained legally required exemption, postponement or spreading overdue payments into installments or suspension of the execution of the decision of the competent authority in full;
3. the Contractor's statement that he has not been the subject of a final court judgment or a final administrative decision on his inability to pay taxes, fees or social security or health insurance premiumsor, in the event of such a judgment or decision being issued, documents confirming the payment of these claims together with any interest or fines or the conclusion of a binding agreement regarding repayment of such claims. the contractor's statement on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and fees (Dz..AT. from 2016, item 716);

I. The Employer will apply the procedure specified in art. 24aa Pzp ("reversed procedure").
II. The offer must be secured by a bid bond in the amount of: PLN 6, 000.00 - III. Initial confirmation of compliance with the conditions of participation in the proceedings and no grounds for exclusion: the Contractor sends in electronic formwith a qualified electronic signature valid as of the day of submitting bids, a declaration in the form of a Uniform European Order Document in the scope indicated in the SIWZ to the email address: [email protected]. The Ordering Party will call the Contractor whose offer will be the highest rated for submitting documents. IV. The ordering party does not provide for supplementary orders referred to in art. 67 sec.1 point 6 and 7 Pzp

VI. Documents and statements from this item are submitted to the ordering party's request under art. 26 par 1 Pzp. Contractor to confirm the ability to perform WES analysis (whole exome sequencing) using genomic DON at least three anonymised results of such analyzes from the last 36 months before the deadline for submission of tenders, and if the period of activity is shorter during this period.
If the Contractor has a registered office or place of residence outside the territory of the Republic of Poland, documents referred to in point III
1) is submitted byan arrangement from a relevant register or, in the absence of such a register, another equivalent document issued by a competent judicial or administrative body of the country in which the Contractor has his registered office or place of residence is the person who is the information or document, to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law issued not earlier than
6 months before u- 2) submits a document or documents issued in the country in which the Contractor has his registered office or place of residence, confirming respectively that his liquidation has not been opened or bankruptcy issued not earlier than 6 months before the deadline for submission of tenders and, that he is not in arrears with paying taxes, fees, social security or health insurance contributionswhether it concluded an agreement with the competent authority regarding the repayment of such receivables, together with any interest or fines, in particular it obtained the legally required exemption, postponement or installment payment of overdue payments or suspension in its entirety the implementation of the decision of the competent authority issued not earlier than 3 months before the deadline for submitting offers.
If in the country, in which the Contractor has his registered office or place of residence, the document is not issued, the documents referred to above are not replaced by a document containing a statement from the Contractor, indicating the person or persons authorized to represent him or a statement of the person which the document was to concern, submitted to the notary or before-a judicial or administrative body or a body of professional or economic self-government appropriate for the seat or place of residence of the contractor or place of residence of the person. - Contractor established in the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, above consists-document in the scope specified in art. 24 sec. 1 points 14 and 21 PPL. If there is no such document in the country in which the person concerned has a place of residence, it shall be replaced by a document containing a statement of that person made before a notary or before a judicial, administrative or professional self-government or economic authority competent forthe residence of that person.
