The subject of the order is: selection of 3 experts in the field of 3 disease groups who will participate in the development of about 25 comments to the weighted atlash risk of a quality coefficient under the project named "Health needs maps Database of System and Implementation Analyzes" co-financed by the European Union from the centers of the European Social Fund under the Operational Program Knowledge Development Education - Expert in the field of endocrine metabolic diseases and diabetes - Expert 1. I.

In order to demonstrate the absence of grounds for exclusion from the procurement procedure in the circumstances referred to in Article . 24 sec. 1 and art. 24 sec. 5 points 1, 2, 3, 4, 8 of the Public Procurement Law, the Employer requires documents referred to in - 5 point-1-6, 9-10, in connection with - 7 para. 1 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the procurement procedure.

II.

The Contractor shall submit in the form of a single document (Single European Order Document - "EAT") a declaration constituting preliminary confirmation that the Contractor is not subject to exclusion and meets-conditions for participation in the proceedings and meets the selection criteria - art. 25a of the Public Procurement Law

Ordering requires submission of EAT with a qualified electronic signature issued by the qualified trust service provider.

The E-OR document created by the contractor must be encrypted (with an access password) by the contractor. For the timely filed EAT is considered the fact of its receipt on the e-mail server in accordance with the official time determined by the Central Office of Measures. More details on how to fold EED through electron communication mediumare specified in the ToR.

III.

Amount of the required bid bond:

Part 1: PLN 50.00 (in words: fifty PLN 00/100),

Part 2: PLN 50.00 (in words: fifty zlotys and 00/100),

Part 3: PLN 50.00 (in words: fifty zlotys and 00/100), - Contractorand he is obliged to pay the bid bond up to the deadline for submitting offers designated in the ToR. The Employer allows any form of payment of the bid bond - in accordance with art. 45 par. 6 Public Procurement Law

IV.

The contracting authority provides for the use of so-called the procedure "reversed" in accordance with Article 24aa of the Public Procurement Law - the Employer does not provide for supplementary procurement, referred to in Article 67 paragraph 1 point 6 and 7 and Article 134 paragraph 6point 3 of the Public Procurement Law Act

Referring to Section IV.2.6): "Minimum period in which the bidder will be bound by the offer" The Contracting Authority informs that in 2 (two months) 60 days should be accepted.

V.

The Contracting Party declares that being The data controller and having regard to the provisions of the Act of 10.5.2018 on the protection of personal data (hereinafter: RODO) implemented security measures adequate to the identifiedrisk and by giving the security of proper protection of personal data transferred as part of the proceedings. A detailed information clause from art. 13 RODO is contained in the SIWZ.
