Construction work. Designing and construction of a fire mast in the Gniezno Forest District, Leśnictwo Hutka "in the design and build formula. Reference Number: S.270.4.2018

1) The subject of the order is" Design and construction of a fire mast in the Forest District Gniezno, Leśnictwo Hutka "in the formula design and build.

Main Site : Address of the construction: Leśnictwo Hutka, Ostrowo, 62-430 Powidz (plot No. 5144/5, precinct Ostrowo, commune Powidz).

3) Under the contract, the future Contractor will be required to execute the order in the following scope - in accordance with the functional and functional program (PF- U) being Annex No. 7 to the Terms of Reference:

a) before preparing an application for a building permit. Contractor foragreement on the construction design with the Purchaser. • b) issue of building conditions for the newly planned mast construction - the Contracting Authority has already acquired the building conditions, - c) reconciliation of the mast location with the Military Unit in Powidz - the Contracting Authority has already obtained the reconciliation - required obstacle marking,

d) creating a map for project purposes covering all elements of the planfor the construction of the mast along with land development and with accompanying teletechnical infrastructure (map for the newly designed mast) - the contracting party has already obtained a map for design purposes in paper version and on an electronic data carrier. e) development of geotechnical documentation in accordance with the regulation on determining geotechnical conditions foundation of building objectszych (Journal of Laws of 2012, item 463), accepting the second category of a construction object (land survey, opinion, geotechnical project) - the contracting party has already obtained geotechnical documentation. f) preparation of a complete project documentation (construction design) necessary to submit application for a building permit for the construction of a new fire-mast structure in accordance with the general guidelines indicated in subparagraphnkcie 3 (PF-U),

g) development of the land development project taking into account the fencing in accordance with the guidelines indicated in point 3 (PF-U),

h) preparation / development of other required arrangements, studies and annexes for completing and submitting to office for the purpose of issuing a building permit, i) preparing an application for a building permit for a planned do mast construction,

j) development of the detailed design,

k) supervision of the author,

l) construction of the mast based on the construction design previously developed and agreed with the investor. Mast should be made in a standard and taking into account the guidelines indicated in point 3 (PF-U).

M) land development in accordance with the development plan developed,

n) executionmast bearing according to the guidelines presented in item 3 (PF-U)

o) preparation of a complete as-built documentation after completion of construction (including protocol of testing and measurements of the lightning protection system, preparation of geodetic survey with vertical measurements and angle of mast shaft torsion), development of a protocol for tightening bolted connections, protocol of force rectification in rope staysattestations, certificates and declarations of conformity of materials used to perform works, etc.),

p) obtaining a use permit for a newly built facility in accordance with the guidelines and indications contained in the final decision on building permit,

q) ordering the area after completion any construction works, including the utilization of waste generated during the process,

3.3. Minimum warranty period required: The Contractor is obliged to provide a minimum of 36-month warranty for materials used and works performedfrom the date of signing the final acceptance report without comments.

In the case of offering a shorter warranty period or failure to provide warranty periods, the Ordering Party will reject the offer under Art. 89 paragraph 1 point 2 of the Act - The warranty period of the parties are determined for the period of the guarantee granted.

The Ordering Party does not specify the condition - the Ordering Party considers the condition to be fulfilled, if the Contractor shows that he hasfinances or creditworthiness not less than: 100, 000.00 PLN

1. Unlimited provisions, procedure based on Art. 24aa Pzp.

2. Criteria for offer evaluation: Price-60% Guarantee period 40%.

3. Contractors who are not subject to exclusion under Art. 24 sec. 1 item 12-23 of the PPL and art. 24 sec. 5 points 1 2 and 4 8 Pzp. The offer must be updated with dthe submission of bids a statement constituting a preliminary confirmation by the Contractor that it is not subject to exclusion and that it meets the conditions for participation in the procedure. The Contractor is obliged to submit the declaration in the form of a Uniform European Order Document. In order to confirm the absence of grounds for exclusion of the contractor from participation in the proceedings, the contracting authority will call the contractor whose offer will be onassessed for the submission of the following documents:

3) excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under art. 24 sec. 5 point 1 of the Act; The ordering party is charged by itself.

4) a certificate from the competent head of the tax office-confirming that the contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submission of bids or other document confirming that the contractor has concluded an agreement with the competent tax authority regarding repayment of such receivables, including any interest or fines, in particular the right to release, postpone or dividepayment of overdue payments in installments or suspension in full of the decision of the competent authority; - 5) certification of the relevant organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions issued earlier than 3 mprior to the deadline for submission of bids or other document confirming that the contractor has entered into an agreement with the competent authority regarding the repayment of such receivables, including any interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension of the full implementation of the decision competent authority;

6) informationand from the National Criminal Record to the extent specified in art. 24 sec. 1 items 13, 14 and 21 PPL and, in respect of convictions for a prison sentence, to the extent specified by the awarding entity pursuant to art. 24 sec. 5 points 5 and 6 of the PPL, issued not earlier than 6 months before the deadline for submission;

7) statements of the contractor about the lack of a final court or final judgment issued against him;an administrative decision on defaulting on payment of taxes, social security or health insurance premiums or, in the event of such a judgment or decision being issued, documents confirming payment of such claims together with any interest or fines or a binding agreement on repayment of such claims; ) the contractor's statement about the lack of a decision(b) the contractor's statement that the contractor has not been the subject of a preventive measure prohibiting him from applying for public orders;

9) a non-final conviction judgment for a restriction of liberty or a fine to the extent specified by the awarding entity on the basis of art. 24 sec. 5 points 5 and 6 of the Act;

10) statements of the contractor about the lack of a final administrative decision against himj on breach of obligations arising from the provisions of labor law, environmental law or social security regulations in the scope specified by the awarding entity pursuant to art. 24 sec. 5 point 7 of the Act;

11) the contractor's statement on not paying taxes and local fees, referred to in the Act of December 12, 1991 on taxes and local fees (Journal of Laws of 2016, item 7)16); The rules regarding the bid bond are set out in Chapter 9 of the SIWZ.
