To confirm no grounds for exclusion Contractors from participation in the Procedure, pursuant to art. 24 sec. 1 item 13-23 (subject to Article 133 (4) of the Public Procurement Law) andparagraph. 5 point 1 of the Public Procurement Law, the Contractor must submit to the ordering party:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of tenders in the Procedure;

2) declaration of the Contractor about the lack of a final court judgment or final administrative decision against himjo arrears of payment of taxes, fees or social security or health insurance premiums or, in the event of such a verdict or decision, documents confirming payment of such receivables with possible interest or fines or conclusion of a binding agreement regarding repayment of such receivables; the lack of a ruling against him as a measure from- an excerpt from the relevant register or central register and information on economic activity, if separate regulations require an entry in the register or in the records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Public Procurement Law - the Contractor declares that in the scope of his economic activityArcza has a liability insurance of at least PLN 1, 000, 000.00 (in words: one million zlotys) and is obliged to maintain insurance in the above-mentioned and in a minimum such amount throughout the term of the Agreement, and provide copies of the policy to the Employer
