The subject of the order is the operation of drainage pumping stations for areas within reach of dammed dams water reservoir Łączany, town: Łączany, poviat of Wadowice, Lesser Poland Voivodeship.

The subject of the order is the operation of pumping stations for drainage of areas within the range of the dammed waters of the Łączany barrage, city: Łączany, Wadowice poviat, Lesser Poland voivodship.

Implementation period 14 months: 1/11/2019. - 31.12.2020

The contractor whose bid was highest rated within the time limitm by the contracting authority, not less than 10 days, consists of:

1) To confirm compliance with the condition described in point 1 point 2 lit. and chapter V of the ToR, a list of services rendered containing information on their subject, values, recipients, dates of performance, together with evidence confirming that these services have been performed properly. A template of the list is Annex 5 to the ToR.

2) In order to confirmfulfillment of the condition described in item 1 point 2 letter b of chapter V of the ToR, a list of persons who will be directed to perform the contract containing information on their professional qualifications, rights, experience and education, as well as the scope of their activities and the basis for their disposal. A template of the list is Annex 6 to the ToR.

3) To be confirmedno grounds for exclusion from the procedure specified in point 1 item 1 of Chapter V of the ToR:

a) a current excerpt from the relevant register or central register and information on the economic activity of the Republic of Poland;

b) current information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before the expiry of the deadlinesubmitting offers in the proceedings;

c) a certificate from the competent head of the tax office confirming that the contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids, or other document confirming that the contractor has entered into an agreement with the relevant tax authority on repayment of these receivables together with any interestor fines, in particular, obtained by law exemption, deferment or installment of overdue payments or suspension of the entire implementation of the decision of the competent authority

d) certificate of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paymentiem contributions for social or health insurance issued not earlier than 3 months before the expiry of the time limit for the submission of tenders, or other document confirming that the contractor has entered into an agreement with the competent authority on the repayment of these claims together with any interest or fines, in particular, he obtained the exemption provided for by law, deferral or installment overduepayment or withholding in full of the decision of the competent authority.

6. The Contractor, who relies on the resources of a third party, in addition to a written commitment to make available to him the necessary resources, submits the documents referred to in point 5 item 4 relating to the third party.

7. Contractor having its registered office or place of residence outside the territory of the Republic of Poland instead of dof documents,

- referred to in point 5 point 4 lit. a and b, respectively:

1) a document issued in the country in which he has his registered office or place of residence confirming that its liquidation has not been opened or bankruptcy has been announced not earlier than 6 months before the deadline for submitting bids;

2) information from the relevant register or, in the absence of such a register, anotherown valid document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or place of residence has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21.
