The subject of the order is: provision of maintenance services, small repair and removal of failures in the roofing industry in real estate owned by the Management Boardnalne Zasobow Lokalowych Sp. z o.o. divided into 2 parts.

The subject of the order is the provision of maintenance services, minor repairs and removal of breakdowns in the roofing industry (divided into 2 parts), which is understood as technical maintenance of real estate in the provision of services providing permanent maintenance of buildings and outdoor areas as well as premises nothose located in these buildings, managed and administered by the Board of Municipal Public Resources Społka z o.o. and communal premises, located in the common housing buildings owned by the Board of Municipal Public Resources Społka z o.o. - for POK 3.

Scope of work includes:

1. The detailed scope of maintenance which is the subject of the order has been specified in the Annexno 1 to Essential Contract Provisions.

2. The Ordering Party reserves the possibility that the quantities and types of works performed, if necessary, may be reduced or increased during the term of the agreement in the event of the takeover by Zarząd Komunalnych Zasobow Lokalowych Sp. z o.o. new real estate in management or transfer of existing real estate to another manager; in the case of the endthe conclusion of a contract with third parties as a result of the Contractor's failure to comply with the order.

3. The list of properties valid as of the date of the announcement of the proceedings - broken down into parts of the orders constitutes Annex No. 8 to the ToR.

The ordering party does not set a specific condition in this respect.

The contracting authority does not set any requirements.

1) The contractor is obliged to bring in beforethe date of submission of offers for the bid bond in the amount of PLN 1, 000 for each part of the order on which the bid is made. In the case of submitting an offer for more than one part of the order, the deposit amount should be added up. In the case of a bid bond in a form other than cash - for example, a guarantee or surety, through the purchasing platform - the Ordering Party requires the submission of a document in electronic form on the principlesspecified in Chapter 20.9 of the ToR with the reservation that it will be signed by a qualified electronic signature by the Guarantor, ie the issuer of the guarantee / guarantee.

2) The Contracting Authority provides for the award of contracts referred to in art. 67 sec. 1 point 6 of the Act consisting in repeated delivery of the same type of services being the subject of the order up to 50% of the value of the basic order.

3) For grantingorders can be claimed by Contractors who are not subject to exclusion from the procurement procedure pursuant to art. 24 sec. 1 and 24 para. 5 point 1 of the Act

4) Pursuant to art. 25a para. 1 and 2 Pzp to the offer, the Contractor shall attach a statement valid at the day of submitting bids in the scope indicated by the Ordering Party in the announcement of the order or in the ToR in the form of a single European documentorders (EAT Annex No. 2 to the Terms of Reference). The Contractor prepares an electronic document by completing the EAT with the help of the ESPD tool or other available tools or software. The contractor is obliged to submit the E-mail in an electronic form with a qualified electronic signature. EAT along with the instructions for its completion will be available on the website of the Purchaser's purchasing platform.In the event of a joint application by contractors, a single document shall be submitted by each of the contractors jointly applying for the order.

5) List of declarations or documents of monies. no grounds for exclusion: a) a) Based on art. 24 sec. 11 Pzp each Contractor, within 3 days from the date of placing on the Procurement platform of the Purchaser information on the opening of bids, referred to inArt. 86 par. 5 PPL, he provides the Ordering Party with a declaration of belonging or non-affiliation to the same capital group (Annex 3 to the Terms of Reference). Along with the submission of the statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure.

B) Current extract from the relevant register or from the central registerand business information, if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion based on Art. 24 sec. 5 points 1 of the Act.

C) Declaration of the Contractor about the lack of a ruling against him as a preventive measure to prohibit applying for a public order - Annex No. 6 to the ToR.

6) In the case of entities acting jointly inthe contractor is obliged to submit the documents specified in point 15.4, 15.5, 15.8, 15.9 of the ToR for each of the entities specified in the agreement regulating the cooperation of entities.

7) If the Contractor, showing compliance with the conditions for participation in the proceedings, referred to in point 13 of the SIWZ, relies on the resources of other entities on the terms specified in art. 22a of the Act, and these entities will participate in the implementationparts of the orders, the Ordering Party requests from the Contractor to present in relation to these entities the documents listed in point 15.8, 15.9 of the Terms of Reference

8) The Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that they refer to their resources, the condition of participation in the proceedings also submits uniform documentsconcerning these entities.

9) If the Contractor has his registered office or place of residence outside the territory of the Republic of Poland, instead of the documents specified in point 15.8 The SIWZ submits a document issued in the country in which it has its registered office or place of residence, confirming respectively that its liquidation has not been opened or that bankruptcy has been declared.
