CPV Description: Refuse disposal and treatment. A service involving the collection and submission of waste recovery code 19 05 03. Reference Number: ZZO-ZZ-341-15-2019. The service consisting in the successive collection for the further development of waste with the code 19 05 03 compost not responding to requirements, in recovery processes. Part one: the task of transport, receipt and management in the recovery process frompank code 19 05 03 in an amount not exceeding 5, 000 Mg, the amount and frequency of receipt will result from the current needs of the Employer.
The subject of the orders includes:
a) Collection of waste with code 19 05 03 compost not meeting the requirements (according to the Regulation Minister of the Environment of 9/12/2014 regarding the waste catalog - Journal of Laws item 1923.) for further developmentin the recovery processes, whereby the receipt of 50% of the indicated mass must take place within 30 days from the date of signing the contract. The remaining quantity and frequency of receipt will result from the current needs of the Employer; - b) development through recovery by the Contractor or other entity indicated by the Contractor who has a permit allowing the processing of these waste in the recovery process in the amount of- waste cited under code 19 05 03;
c) waste with code 19 05 03 is an unfractionated compost produced from waste biodegradable, deprived of ferrous metals, coming from a selective collection. However, it may still be unstable in terms of oxygen demand and organic matter content. Masoin about 95% of the state organic waste, while the remaining part in quantity - massively can constitute 1.5% mineralized pieces of wood in the size of the area. 19 mm, 2% glued wood and dried compost, 1% mineral fraction stones and debris, 0.5% plastic film and materials, non-ferrous metals, glass;
d) Confirmation of receipt and submission of waste recovery code 19 05 03 ;
e) transportt waste of code 19 05 03 from Zakład Zagospodarowania Odpad-w in Poznań Sp. z o.o. from the Biocompostownia location, ul. Meteorite 3 for the installation of the Contractor (or entity ultimately managing the waste being the subject of the contract) is provided by the Contractor. The Contractor ensures unloading of the Contractor (or entity ultimately managing the waste constituting the subject of the contract) on the site of the installation., The Ordering Party provides loading at the Biocomposting installation. The loading will be carried out from Monday to Friday from 7:00 to 20:00 and on Saturdays from 7:00 to 13:00;
f) The Ordering Party will provide the Contractor by phone or via email or in writing information on the need to manage waste on the part of the Employer together with their quantity;
g) The frequency of waste collection will depend on the Ordering Party's needs;
h) the Ordering Party and the Contractor will determine the persons to coordinate the contract indicating the name and surname, e-mail address and phone number to contact;
) The Contractor is obliged to collect waste 19 05 03 within 3 days from the time of notification by the Employer. - The Contractor has the competence or the right to conduct a specific plotProfessional activity covered by the present proceedings.
In the scope of task 1: - the Contractor has the rights or permits, ie appropriate permits for transport and processing (in an amount not less than 5, 000 Mg per year) by recovering waste with code 19 05 03.
Contractors jointly applying for the award of a contract (eg a consortium) may fulfill this condition jointly (it is notit is necessary for each contractor to have permits to collect, process and transport waste.) Within the scope of task no. 2: - the Contractor has the rights or permits, ie appropriate permits for transport and processing (in an amount not less than 2, 500 Mg per year) ) by recovering waste with code 19 05 03. - Contractors jointly applying for a contract (eg a consortium) present inthe condition can be met jointly (it is not required that each contractor would have permission to collect, process and transport waste). Within the scope of task no. 3: the Contractor has the rights or permits, i.e. the appropriate permits for transport and processing (in the quantity less than 2, 500 Mg per year) by recovering waste with code 19 05 03. Contractors jointly applying for grantingordering (eg consortium) this condition can be fulfilled jointly (it is not required that each contractor has permission to collect, process and transport waste).
The Contractor is excluded from the proceedings under Art. 24 sec. 1 point 12 23, and par. 5 point 1 Pzp. In order to certify that there are no grounds to exclude the Contractor from the proceedings for the order in ca.the frequencies referred to in art. 24 of the Act, the Contractor is obliged to submit:
a) Information from the National Criminal Record issued not earlier than 6 months before the deadline for submitting offers;
b) Declaration of the contractor about the lack of a final court decision or final administrative decision on default of tax payment, fees or social insurance premiumsor, in the event of such a judgment or decision being issued, documents confirming the payment of such claims together with any interest or fines or the conclusion of a binding agreement on repayment of such debts; c) Declaration of the Contractor about the lack of a ruling against him as a precautionary measure apply for public orders;
d) Copy from inclproper register or from central records and information about business activity, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act;
e) Contractor's statements about affiliation with or lack of belonging to the same capital group; in the case of affiliation to the same capital groupthe contractor may submit together with the declaration documents or information confirming that connections with another contractor do not lead to distortion of competition in the procedure.
Each contractor is required to submit the following declarations and documents confirming the fulfillment of the conditions for participation in the proceedings:
a) Single European Order Document (offer);
b) Documents confirming ownershipcompetences and rights (submitted to the Ordering Party's request pursuant to Article 26 (1) of the Act). The Contractor is obliged to pay a bid bond in the amount of: - PLN 1, 000.00 within the scope of task No. 1;
- PLN 1, 000.00 within the scope of task No. 2;
- PLN 1, 000.00 within the scope of task no. 2.
The bid bond should be submitted before the deadline for the submission of offers, whereby the payment of the deposit in cash by means of a transferThe Ordering Party shall consider it effective only if before the deadline for the submission of tenders the amount of the security lodged will be credited to the Contracting Party's bank account. It is recommended that a copy of the proof of payment of the bid bond be included in the offer.
3. The bid bond can be paid in one or more of the following forms: a) money: by bank transfer to the Employer's bank account: 53 1050 1520 1000 00234950 8362;
b) bank guarantees or social savings and credit union guarantees, the cash security guarantee is always a financial guarantee;
c) bank guarantees;
d) insurance guarantees;
e) sureties granted by entities referred to in art. 6b par. 5 points 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development.

5. The tender deposit made in a form other than in money must be valid at least until the end of the contracting period of the offer submitted by the contractor.
6. In the case of payment of a bid bond in a form other than money, an appropriate document should be attached to the offer, unbuttoned with the offer.
