Collection and management of municipal waste from property owners residing in the commune of Olesno
Reference Number: 271.33.2019
3.1. The subject of the order is the implementation of the service of collecting and managing municipal waste from property owners, where residents of the Olesno commune live and the management of collected municipal waste from the Selective Municipal Waste Collection Point and equipping the Point with containers.

The subject of the contract is the implementation of the service of collecting and managing municipal waste from the owners of the property where they reside residents onwithin the commune of Olesno and management of collected municipal waste from PSZOK and equipping the Point with containers.
As part of the contract, the Contractor is obliged to:
1) Collect and use the following types of municipal waste from property owners:
a) mixed municipal waste,
b) selectively collected waste: plastic and metal wastes (metastatic wastesali, including metal packaging waste, plastic and metal waste, paper and cardboard waste, glass waste,
c) biodegradable municipal waste (bio waste and green waste),
d) waste electrical and electronic equipment,
e ) bulky waste (furniture, sofas, wardrobes, chairs, tables, rugs, baby carriages, mattresses, duvets, doors, windows, large-size toys, bicycles, Christmas trees, car tires, etc.)
2) Equip the Selective Municipal Waste Collection Point with containers and collect and manage the following types of municipal waste from PSZOK:
a) waste from paper and cardboard,
b) metal,
c) plastic waste (plastic bottles, cosmetics and cleaning products bottles) and multi-material packaging,
d) glass waste,
e) green wastee,
f) expired drugs,
g) chemicals (paints, solvents, waste oils),
h) used batteries and accumulators,
i) fluorescent lamps,
j) used electrical and electronic equipment,
k) bulky waste,
l) construction and renovation waste up to 400 kg per person in the household,
ł) debris,
m) worn tires,
n) waste resulting from the receipt of medicinal products in the form of othermonitoring and blood monitoring, in particular needles and syringes (ex 20 01 99).
3) Provide property owners with appropriate types and quantities of containers or bags for collecting waste that meets the requirements set out in the "Regulations for maintaining cleanliness and order on within the commune of Olesno ", on terms agreed between the owner of the propertyand the export company and the rules set out in:
Resolutions No. XL / 305/17 and Resolution No. XL / 306/17 of the City Council in Olesno of October 9, 2017
4) List of real estate and waste collection points (dump arbors), covered by the subject of the contract, which should be equipped with containers / containers, sacks, their type and quantity, the Employer shall provide the Contractor before concluding the contract foror and waste management.
5) The Employer during the contract period with a frequency of once a week undertakes to inform the Contractor about changes in the data included in the list referred to in point 4.
6) In the event of changes in the list referred to in point 4, resulting in its extension by further real estate or waste collection points (garbage cans) not included in the list, Contractor withwill be required to:
a) equip the property with containers or bags,
b) collect municipal waste from these properties, starting from the week following the week in which the information on submitting the declaration was included in the list.
7) In the event of changes in the list, referred to in item 5, resulting in a reduction of the list of real estate or waste collection points included in the list, wca will be obliged to:
a) collect the container (s) from these properties,
b) terminate the provision of municipal waste collection services from these properties, starting from the week following the week in which he received information on removal from the list.
8) Wash and disinfect containers for mixed and biodegradable waste belonging to the Contractor, delivered to the propertywithin the commune of Olesno as part of the main order.
A detailed description of the subject of the order has been set out in the Annex to the ToR. utilities from property owners kept by the Mayor of Olesno,
2a) entry to BDO regarding waste transport,
3a) entry to BDO for the collection of waste electrical and electronic equipment,
4a) have a valid permit for recovery or disposal of municipal waste subject to the order issued on the basis of the Waste Act, if the Contractor intends to recover or disposal of waste on its own. In caseawca intends to transfer waste to another entity for recovery or neutralization of the contract for the indicated scope with the entity authorized in this respect,
5a) has an entry in the Registry of Products and Packaging and Waste Management Database (BDO), created on the basis of the provisions of the Act of waste on December 14, 2012.
List of declarations or documents submitted by Execthe adviser whose offer was highest rated in the proceedings, at the request of the Employer to confirm the circumstances referred to in art. 25 paragraph 1 point 1 of the Public Procurement Law:
a) concession, permit, license or document confirming that the Contractor is entered in one of the professional or trade registers kept in the European Union Member State in which the Contractor hasor place of residence;
the Employer does not specify in this respect any conditions that the Contractor is obliged to demonstrate in a special way
In connection with the limitation of the number of characters in individual sections of this announcement, the detailed scope of the contract has been specified in the Terms of Reference.
The Contractor is obliged to bring a deposit of PLN 20, 000 (say: twenty thousand zlotys)zlotys). The detailed requirements for the tender bond are set out in the Terms of Reference. The contractor is excluded from the contract award procedure in relation to which any of the circumstances referred to in art. 24 paragraph 1 point 12 23 of the PPL Act. List of statements or documents submitted by the Contractor, of whicherta was the highest rated in the proceedings, at the request of the Employer, to confirm that he was not subject to exclusion:
- information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,
- the Contractor's statement that he has not issued a final or final judgment against himadministrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the event of such judgment or documentary evidence confirming the payment of such receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of these receivables,
- statement of the Contractor on no judgmentbecause of it as a precautionary measure the prohibition to apply for public contracts; if the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the document referred to above (information from KRK), it submits a document issued in the country in which it has its registered office or place of residence, issued not earlier than 6 months before the deadline for submitting offers, submits informafrom the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or place of residence, has the person to whom the information or document relates, in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act. If in the country in which the Contractor is seatedthe person or place of residence or place of residence has the person to whom the document relates, does not appear the document referred to above, it is replaced with a document containing, respectively, a statement of the Contractor, with an indication of the person or persons authorized to represent it, or a statement of the person whose document relate, submitted to a notary or judicial authority, administrator a professional or economic self-government body competent for the seat or place of residence of the Contractor or the place of residence of that person.
