Collection and management of municipal waste from owners of real estate residing in the commune of Olesno.

3.1 The subject of the contract is implementation municipal waste collection and management services from property ownersestate, where residents in the commune of Olesno reside, and management of collected municipal waste from the Selective Municipal Waste Collection Point and equipping the point with containers.
Main Site: commune of Olesno
As part of the contract, the Contractor is obliged to:
1) Collect and use the following types municipal waste from property owners:
a) mixed municipal waste,
b) selectively collected waste: plastic and metal waste (metal waste, including metal packaging waste, plastic waste, including plastic packaging waste, and multi-material packaging waste), waste from paper (paper waste, including cardboard, paper and cardboard packaging waste) glass waste (colored and colorlessglass waste, including glass packaging waste), biodegradable municipal waste (bio waste and green waste),
d) waste electrical and electronic equipment,
e) bulky waste (furniture, sofas, wardrobes, chairs, tables, carpets, baby carriages, mattresses, duvets, doors, windows, large size toys, bicycles, Christmas trees, car tires etc.)
2) Equip Selective Collection PointMunicipal waste in containers included in point 3 of the description of the subject of the order and collect and use the following types of municipal waste from PSZOK:
a) waste from paper and cardboard,
b) metal,
c) plastic waste (type of plastic bottles, bottles cosmetics and cleaning products),
d) glass waste (colored and white),
e) green waste,
f) expired drugs,
g) chemicals (paints, solvents, waste oils),
h) used batteries and accumulators,
i) fluorescent lamps,
j) used electrical and electronic equipment,
k) bulky waste,
l) construction and renovation waste up to 400 kg per person in the household,
ł) debris,
m) used tires,
n) syringes and needles.
3) Provide property owners with appropriate types and quantities of containers for real estateb bags for collecting waste meeting the requirements set out in the "Regulations for maintaining cleanliness and order in the commune of Olesno", on the terms agreed between the owner of the property and the export company and the principles set out in:
a) Resolution No. XL / 305/17 of the Municipal Council in Olesno of October 9, 2017 regarding the adoption of the Rules for maintaining cleanliness and order (...)
b) ResolutionNo. XL / 306/17 of the City Council in Olesno of October 9, 2017 regarding the detailed manner and scope of rendering services in the field of municipal waste collection (...) 4) List of real estate and waste collection points (garbage canopies) covered by the subject orders that should be equipped with containers / containers, their type and quantity, the Purchaser will providebefore concluding the contract for the collection and management of waste.
5) The Employer during the contract period with a frequency of once a week undertakes to inform the Contractor about changes in the data covered by the list referred to in point 4.
6) In the event of changes in the list, referred to in point 4, resulting in its extension with further real estate or waste collection points (garbage cans)not included in the list, the Contractor will be obliged to:
a) equip the property with containers or bags,
b) collect municipal waste from these properties, starting from the week following the week in which the information on submitting the declaration was included in the list.
7) In the event of changes to the list referred to in point 5 resulting in a reduction of the list of immovable property or collection pointswastes included in the list, the Contractor shall be obliged to:
a) collect the container (s) from these properties,
b) terminate the provision of municipal waste collection services from these properties, starting from the week following the week in which he received information on removal from the list.
8) Wash and disinfect mixed and biodegradable waste containers belonging to Wykof the ordering party, delivered to the property in the commune of Olesno as part of the main order (...)
A detailed description of the subject of the order has been set out in the Annex to the ToR. to the register of regulated activities in the field of collecting municipal waste from property owners keptby the Mayor of Olesno,
2a) have a current permit for the transport of municipal waste subject to the order issued on the basis of the Waste Act,
3a) have a current entry in the register of waste electrical and electronic equipment collection kept by the Chief Inspector of Environmental Protection or ensuring readiness to accept waste entrepreneur with an entry, referred to above,
4a) have a current permit for recovery or disposal of municipal waste subject to the order issued on the basis of the Waste Act, if the Contractor intends to carry out recovery or disposal of waste on its own. If the Contractor intends to transfer waste to another entity for recovery or neutralization of contractsę on the indicated scope with the entity authorized in this respect.
5a) has an entry in the Database of Products and Packaging and Waste Management (BDO) Registry, created on the basis of the provisions of the Act of 14 December 2012 on waste.
List statements or documents submitted by the Contractor, whose tender was the highest rated in the proceedings, at the request of the Employer into confirm the circumstances referred to in art. 25 paragraph 1 point 1 of the Public Procurement Law:
a) concession, permit, license or document confirming that the contractor is entered in one of the professional or trade registers kept in a Member State of the European Union in which the contractor has its registered office or place of residence
The contracting authority does not specify in this respect, no conditions, whichThe Contractor is required to demonstrate compliance in a special way
In connection with the limitation of the number of characters in individual sections of this announcement, the detailed scope of the order has been specified in the Terms of Reference. Detailed requirements regarding the bid bond are specified in the ToRsecurity requirements are specified in the SIWZ.
The offer should be made in the Polish language, in electronic format in the form of publicly available data and signed by a qualified electronic signature.
The Single European Order Document should be attached in the electronic form with a qualified electronic signature, and then together with fileswhich compress the offer into one archive file (ZIP)
The contractor is excluded from the contract award procedure in relation to which any of the circumstances referred to in art. 24 paragraph 1 points 12-23 of the PPL Act. List of declarations or documents submitted by the Contractor, whose bid was highest rated in the proceedings, at the request of the Employer, to confirme subject to exclusion:
- information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of bids,
- the Contractor's statement that he has not issued a final court judgment or the final administrative decision regarding arrears with the payment of taxes, fees or insurance premiumssocial or health conditions, or in the event of the issuance of such a judgment or a decision confirming the payment of such receivables together with any interest or fines, or the conclusion of a binding agreement on the repayment of those receivables, public procurement; ifthe contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the document referred to above (information from KRK), submits a document issued in the country in which it has its registered office or place of residence, issued not earlier than 6 months before the deadline for submission of tenders, submits information from an appropriate register or, in the absence of such a register, another equivalent documentnt issued by the competent judicial or administrative authority of the country in which the contractor has its registered office or place of residence or place of residence has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act. Dock. should be issued no earlier than6 months before the submission deadline.
