1 The proceedings are conducted in an unlimited tender. Communication between the Employer and Contractors (including submission of offers, statements and other correspondence) will take place in aEditing the SmartPZP Purchasing Platform at: https://portal.smartpzp.pl/kgsg 3. The contractor will provide the model of the subject of uniforms to the Ordering Party (to the Office of the Information Security Office of KGSG in Warsaw at 38 Podchorążych Street, building No. 5, room 42 tel. number on the city line: 48 225004372 or 48 225005337, from the office of passes on the departmental line 76604372 or76605337) within the time limit set for submitting bids. 4. The order covered the supply of: 1) khaki-colored output jackets (part 1 orders); 2) business jackets in olive color (part 2 orders), 3) clothes jackets for bad weather with a lining (part 3 orders). 4. As part of the execution of each part of the order, the Contracting Authority provides for the option to use the option.

1. The subject of part 1 of the order is the delivery of 540 kpl output jackets in khaki color, including: a) delivery of 440 kpl. above. jackets - as part of the basic order; b) delivery of a maximum of 100 sets above. jackets - as part of the order subject to the law options. 2. The contractor is obliged to do sost to deliver, within the deadline set for submission of offers, the final design of the uniform item offered in the first grade of quality, ie: khaki-size starting jacket in size: 175 cm height, chest circumference 98 cm (1 set). 3. The Employer acting pursuant to art. 24aa of the Public Procurement Law, will first evaluate the offers (including verification of the model of the exit jacket offered inkhaki supplied with the offer), and then will examine whether the Contractor, whose offer has been evaluated as the most advantageous, is not subject to exclusion and meets the conditions for participation in the proceedings. 4. The Ordering Party will call the Contractor whose offer has been the highest rated, to submit within the prescribed period, not shorter than 10 days, valid as of the date of submitting the below-mentioned documentsstating that the offered deliveries meet the requirements of the Ordering Party: 1) analyzes, attestations, test reports or other equivalent documents containing technical and utility indicators of the main fabric, knitted fur collar, synthetic lining used to manufacture the starting jacket, confirming the fulfillment of the requirements contained in the description order subject matterimplementation of Part 1 of the contract, these documents must be issued by: accredited testing laboratories or manufacturers producing those materials; 2) one copy of the technical and technological documentation regarding the production of the uniform item offered in the Polish language version, containing at least the following information: a) description of the subject of the order, b) drawing (drawings), c) technical requirements including, among others: a list of components, a list of materials and accessories necessary to perform the order, d) a description of how to order and its designation, e) a table of dimensions, f) a description of the packaging method and storage. The documents indicated in point 1) must be submitted in the original in the form of an electronic documentprovided with a qualified electronic signature of the issuer of the document or an electronic copy of a document certified by the Contractor for compliance with the original, whereas the document indicated in point 2) must be submitted in the form of an electronic document with a qualified electronic signature by the Contractor or an electronic copy of a document certified by Wthe Contractor for compliance with the original, whereby the certificate confirming the conformity to the original of the electronic copy of the document is made using a qualified electronic signature of the person / s authorized to represent the Contractor. 5. Whenever the subject of the order is described by indicating: 1) trade marks (trade name), 2) standards, the Employer accepts the use of equivalent solutions, which should be understood in relation to: point 1) products that meet the requirements of the Employer at least in the same scope as the products described in the description of the subject of the order (technical specification of the product); to point 2) solutions related to the research methodology of materials and the finished product, in which tests will be carried out according to revised standards or other research procedures, oThe applied methodology will be the same as the methodology of the standard indicated by the Ordering Party in the description of the object of the order (technical specification of the product). The Contractor invoking the equivalent solutions described by the Ordering Party is obliged to demonstrate in the offer that the subject of the order he offers meets the requirements specified by the Ordering Party. For ordering, pleaseto apply for a Contractor who is not subject to exclusion from the proceedings under Art. 24 sec. 1 point 12) 23) of the Public Procurement Law and art. 24 sec. 5 point 1) of the Public Procurement Law and meets the conditions for participation in the proceedings specified in point III.1.3) of this announcement. 2. In order to pre-confirm that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings, the Contractor is obliged todeliver together with the offer valid as at the day of submitting bids a statement in the form of a single document drawn up in accordance with the standard form provided in the Commission Implementing Regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU and art. 80 par. 3 of Directive 2014/25 / EU, hereinafter referred to as the Single European Order Document or "EAT".The ordering party will call the Contractor, whose offer has been rated the highest, to submit within the prescribed period, not shorter than 10 days, valid as of the day of submitting the nw. documents: a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of offers; b) Contractor's declaration for bto issue to him a final court judgment or final administrative decision on the late payment of taxes, fees or social security or health insurance premiums or in the event of issuing such a judgment or decision documents confirming the payment of these claims together with any interest or fines or the conclusion of a binding agreement in on repaymentthese receivables; c) declaration of the Contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for a public order; d) excerpt from the relevant register or central records and information about business activity, if separate regulations require an entry in the register or records, in order to confirm the lack of grounds for exclusion under Article. 24 sec. 5 point 1PPL Acts; e) list of deliveries and evidence referred to in section III.1.3). 4. In addition, the Contractor must submit within 3 days from the date of placing by the Employer on the website information referred to in Article. 86 par. 5 of the Public Procurement Law, declarations of belonging or non-affiliation to the same capital group, referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. 5. Offer and axisProvision in the form of "EAT", referred to in Article 25a of the Public Procurement Law, is made, under pain of nullity, in electronic form and is provided with a qualified electronic signature of the person / s authorized to represent the Contractor. referred to in the Regulation of the Minister of Development of 26.7.2016 on the types of documents that may be requested by orderingy from the Contractor in the procurement procedure (Journal of Laws of 2016 item 1126) and in the Regulation of the Minister of Entrepreneurship and Technology dated 16/10/2018 amending the regulation on the types of documents that the Employer may request from the Contractor in the proceedings for the award of a contract (Journal of Laws of 2018, item 1993) (ie documents and statements listed in item 3a) 3e) and in. 4) are submitted in the original in the form of an electronic document or in an electronic copy of the document or a statement certified as true to the original. 7.
