1. The subject of the order is the adaptation of the electronic composition for printing and printing - "Closer to Brussels" magazine dedicated to the Awangarda Initiative and delivery.

Main Site: The contractor will provide printed-magazine for the headquarters of the Malopolska Province Representation Office in Brussels at: South Poland House Małopolska, rue de la Science 41, 1er etage, B-1040 Bruxel

1. The subject of the order is the adaptation of the electronic composition for printing and the printing of the e-magazine "Closer Brussels" devoted to the Avangarda Initiative and delivery

2. The print and binding of the warehouse should meet, among others, the following assumptions(a) Size: format 210 mm (width) x 297 mm (height) A4;

b) Number of pages: 24-28 pages cover (4 pages);

c) 250 copies, including 150 pieces in English language and 100 items in Polish.

3. The Contractor will deliver a printed magazine to the headquarters of the Malopolska Province Representative Office in Brussels to the following address: South Poland House Małopolska, rue de la Science 41, 1er etage, B-1040 Bruxelles, Belgium.

4. The description of the subject of the order and the conditions for the execution of the order are contained in the contract template constituting Annex 6 to the ToR.

5. The publication does not have an ISBN / ISSN number.

I.1. Contractors can apply for the contract, who:

a) are not subject to exclusion pursuant to art. 24 sec. 1 of the Act and art. 24 sec. 5 point 1 and point 8 of the Act,

b) meet the conditionTo participate in the proceedings regarding technical or professional capacity:

The condition of participation in the procedure will be considered fulfilled when the Contractor has experience in proper implementation (or in the performance of periodical or continuous services) within the last 3 years before the deadline for submission of offers and if the period of business is shorter during this period, at least two services corresponding to the type of service that is the subject of the order:

the service of composition and printing is considered to be one service corresponding to the type of service constituting the subject of the order.

2. The Contractor, in order to confirm the fulfillment of the condition of participation in the proceedings, may rely on the capacities of other entities under the terms specified in art. 22a of the Act

3. ContractorsThose who apply for the award should appoint an attorney to represent them in the proceedings or represent them in the proceedings and conclude a public procurement contract (the offer must be accompanied by a relevant power of attorney), unless in the case of a civil partnership, the contract provides a way to represent it ( to determine what it will be necessary to attach to oferty civil partnership agreement). All correspondence and settlements will be made only with the acting entity as the representative of the remaining ones. The offer must be signed in such a way that it legally obliges all entities acting jointly. The Contractors jointly applying for the contract bear joint and several liability for the performance of the contract.

II. List of statements or documents potwho meet the conditions for participation in the proceedings and no grounds for exclusion - II.1) Declarations and documents submitted with the offer:

1. The contractor should enclose the current statement on the form of a single European order document (hereinafter the EAT declaration), which model is attached as Annex 2 to the Terms of Reference. The information contained in the statementDZ constitute preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the procedure.

The contracting authority informs that the contractor in EED may limit himself to completing section α (alpha): General statement regarding all eligibility criteria in Part IV and does not have to fill in any from the remaining sections in part IV. GOOD

Note! The Ordering Party encloses the form JEDZ / ESPD in .xml format as Attachment 2 to the ToR and informs that in order to supplement it, the attachment should be downloaded from the Ordering Party's website (BIP), saved on the disk, and then imported and supplemented by the JEDZ / ESPD dos serviceat the following address:

The EAT form should be handed over to the Ordering Party in the form referred to in point VIII.1 SIWZ (ie EAT should be sent in an electronic form with a qualified electronic signature). Please note that the ESPD service does not archive files.

2. In the case of joint application by contractors forstatement, declaration of the EAT shall be submitted by each of the contractors jointly applying for the order.

3. Contractor who refers to the resources of other entities on the terms specified in art. 22a of the Act, in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that they refer to their resources, the condition of participation in the proceedings also consists of a uniform European document-orders related to these entities

4. If the Contractor, in order to confirm fulfillment of the condition of participation in the proceedings, relies on the capacities of other entities on the principles specified in art. 22a of the Act:

1). must prove to the orderer that, when executing the order, he will have the necessary resources of these entities, in particular by presenting the obligation of these entities to return to himdetermination of necessary resources for the needs of the order. The Awarding Entity recommends making an undertaking according to the template constituting Annex 5 to the ToR.

5. In the event that the Contractor is represented by the Proxy together with the offer, a power of attorney should be submitted for that person specifying its scope. The power of attorney should be signed by persons authorized to represent the Contractor.

6. All plenipotentiariesshould be attached to the offer in the form of an original or officially certified copy of the power of attorney (notarial art 96 of the Act of 14.2.1991 Notary Public Law / ie OJ of 2014, item 164 with later amendments / II.2) Declaration of belonging or non-affiliation to the same capital group:

1. In order to confirm the absence of grounds for excluding the contractor from participation in the procurement procedurerequests the contractor's declaration of affiliation or non-affiliation to the same capital group; in the case of belonging to the same capital group, the contractor may submit together with the statement documents or information confirming that connections with another contractor do not lead to distortion of competition in the procedure. Contractor, within 3 days of placing by Orderon the website of information referred to in art. 86 par. 5 of the Act, is obliged to provide the Employer with the statement referred to in para. 1

3. The statement is made in the original.

4. Model statement along with the information referred to in art. 86 par.5 of the Act, will be posted by the Employer on the website on which he was involved SIWZ with attachments.

II.3) Statements and documents confirming the circumstances referred to in Article. 25 ust. 1 submitted to the ordering party's request:

1. The contracting authority will call for submitting documents and statements:

1). confirming the lack of grounds for exclusion from participation in the proceedings:

a). inforfrom the National Criminal Register to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting offers;

b). certificate of the competent head of the tax office confirming that the contractor is not in arrears with paying taxes, issued not earlier than 3 months before the deadline for submission of bids, or other documentationhere confirming that the contractor has entered into an agreement with the relevant tax authority regarding the repayment of such claims together with any interest or fines, in particular it has obtained the legally required exemption, deferral or payment in installments of overdue payments or suspension in full of the decision of the competent authority; certificate of the relevant organizational unit ZaSocial Insurance Fund or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the contractor has concluded an agreement with the competent authority regarding the repayment of these amountswith particular interest or fines, in particular obtained legally required exemption, postponement or payment in installments of overdue payments or suspension in full of the execution of the decision of the competent authority;

1. The offer (each document making up the offer) must be drawn up in Polish, subject to written form.

2. Before the deadline for submitting offers, the contractor fromis obliged to submit a bid bond, for the duration of the period of validity of the bid in the amount of: PLN 90.00 in words: ninety zlotys.

3. The ordering party in accordance with art. 24aa of the Act informs that it will first evaluate the offers and then examine whether the contractor whose offer has been evaluated as the most advantageous, is not subject to exclusion and meets the conditions for participation in the procedure. In progressivethe statement shall be made in writing or in electronic form, however, the EAT shall be sent in an electronic form bearing a qualified electronic signature.
