Provision of postal services for OR AMW in Krakow

The subject of the contract is the provision of postal services for the Regional Department of the Military Property Agency in Krakow in domestic and foreign trade in the scope of:

Postal items are understood as unregistered letter items, registered in S format up to 500 g, M dby 1, 000 g, L to 2, 000 g, postal parcels over 2, 000 g.

The Purchaser undertakes to prepare parcels for posting and quantitative lists of unregistered parcels and lists specifying the recipients of registered parcels and to place information on parcels in a permanent and legible manner uniquely identifying the addressee and sender, specifying the type of shipments (ppriority recommended, with return confirmation of receipt), full name and return address of the sender and placing on the address page of shipments a sign confirming payment for the service.

The shipments will be collected by the Contractor in Krakow, ul. Montelupich 3, every business day from 8:00 10:00.

The Contractor shall deliver to the Purchaser's headquarters each shipment addressed to himbusiness day (Mon-Fri) from 8:00 10:00, excluding public holidays. Sending parcels covered by the subject of the order will take place on the day they are delivered by the Purchaser, if there are no objections to the parcels.

The total maximum weight of shipments transferred will not exceed 100 kg at one pickup, while the weight of one collective package will not bee exceeded 20 kg.

The Contractor shall provide the Purchaser with a return receipt acknowledged by the addressee, whose delivery takes place under the terms of the Code of Administrative Procedure, immediately after delivery of the package.

In the absence of the addressee, the Contractor shall leave a notice (first notification) delivery of an item with an indicationwhere and when the recipient can collect the parcel. The deadline for receiving the parcel by the addressee is 14 days from the day following the day of leaving the first notification, within this period the parcel is notified twice. After the deadline for receipt, the parcel is returned to the Ordering Party together with the reason for not collecting it by the addressee.

The number of parcels of a given type will be sent successivelynot throughout the duration of the contract, and their number during the contract period will depend on the needs of the Employer.

The basis for settling the costs of services provided will be the sum of fees for shipments actually sent and returned within one month.

Payment for services rendered will be made in arrears monthly billing period.

The contractor should meet the requirements for registrationsin terms of effective and correct delivery, in accordance with applicable provisions: Postal Law, Code of Civil Procedure, Code of Criminal Procedure, Code of Administrative Procedure, Tax Code, Public Procurement Law. to such an extent that without changing istservices, especially prices, adapt the contract to the amended acts.

The prices specified by the Contractor in the offer will be reduced during the implementation of the contract if the postal charges resulting from the standard price list or the Contractor's regulations are lower than the prices resulting from the offer. The contractor is then obliged to apply reduced postal charges resulting from its current amountabout the price list or regulations.

In the event of a reduction in prices, the Contractor is obliged to provide the Employer with a new price list, which will be valid until the end of the contract.

on the proof of posting the shipment there will be an entity other than the Employer. UploadingPatches must be sent only on the basis of documents and printed proofs of posting, delivery and receipt completed by the Employer. On the envelopes, documents and forms must include the address of the Employer. The Employer must appear on them as the sender.

In the case of Contractors jointly applying for the award of orders, JEDZ shall submit each of the Contractors, in which each of the Contractors shows no grounds for exclusion;

The Employer does not require the submission of a JESZ regarding subcontractors.

Before awarding the contract, the Employer will call the Contractor whose bid is the highest rated to submit valid statements or documents confirming the lack of grounds for exclusion from the procedure and compliance with the conditions for participation in theproceedings (in the case of Contractors jointly applying for the award of the above-mentioned order, statements and documents are submitted by each of the Contractors acting jointly):

1) in relation to the circumstances specified in art. 24 paragraph 1 of the Act:

a) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act issued not earlier than 6 months before the expiry ofthe bid submission deadline;

b) the Contractor's statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or in the case of such a judgment or decision of documents confirming the payment of such debts or conclusion of a binding agreement onnot repayment of these receivables, prepared in accordance with the specimen constituting Annex 4 to the ToR;

c) the Contractor's statement of the lack of a decision against him as a preventive measure prohibiting him from applying for public contracts, prepared according to the specimen constituting Annex 5 to the ToR;

2) in order to confirmation of compliance with the conditions for participation in the proceedings

a) entry in the register of postal operators, prconducted by the President of the Office of Electronic Communications.

The above-mentioned statements and documents, the Contractor whose tender has been rated the highest, will be obliged to submit, at the request of the Employer within a specified time limit, not less than 5 days.

The Contractor shall be obliged to provide a deposit of 5, 000.00 PLN (five thousand zlotys).

When selecting the best offery The Employer will be guided by the following criteria:

a) Gross price of the offer, weight 60%,

b) Number of contractor's outlets in which you can collect parcels advised in the following provinces: Małopolskie, Śląskie, Świętokrzyskie and Ł-dź (only Tomasz-w poviat), weight 40%.

In order to prove that there are no grounds for exclusion, within 3 days of placing on the website platforms https://amw.eb2b.com.pl, information referred to in art. 86 section 5 of the Act, the Contractor shall provide the Ordering Party with a declaration of belonging (together with evidence confirming that connections with another Contractor do not lead to distortion of competition in the proceedings) or lack of belonging to the same capital group as referred to in Art. 24 paragraph 1 point 23 of the Act, prepared according to the model staof the new Annex 6 to the ToR.

If the offer is signed on behalf of the Contractor by a person other than authorized to represent the Contractor, the offer should be accompanied by a document or documents from which the scope of authorization will arise.

In accordance with art. 13 section 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 on the protection of individuals with regard tobrewing personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation) (OJ EU L 119, 4.5.2016, p. 1), I inform that the personal data administrator is the Agency Mienia Wojskowy with headquarters at ul.
