Supply and installation of automated measurement systems for physical and chemical indicators of water quality operating on-line.

Main Site: Places of implementation indicated in the section II.2.4. announcements. - Delivery and installation of automated measuring systems for physical and chemical indicators somehowof water operating in on-line mode.

The subject of the order is:

1. delivery, installation, calibration and commissioning of 8 automated measuring systems for physical and chemical indicators of water quality operating in on-line mode (hereinafter: Test Stations) in the following locations:

a. 2 Research Stations allowing for the control of water quality in on-line mode of the following indicators:water enamel in 254 nm, pH, redox potential (ORP), temperature, electrical conductivity, OWO, DOC, nitrates, total chlorine (installation for SUW Maczki - Maczek Water Treatment Station, Wodociągi St. 4, 41-217 Sosnowiec, SUW Kozłowa Gora - Kozlowa Gora Water Treatment Station, 34 Leśna St., 42-584 Wymysłow),

b. 1 Research Station allowing for water quality control in on-line modeindicators: water permeability in 254 nm, pH, redox potential (ORP), temperature, OWO, DOC, nitrates, total chlorine (installation at SUW Czaniec - Czaniec Water Treatment Station, Wodociągowa St. 1, 43-356 Kobiernice),

c. 4 Research Stations allowing on-line water quality control in the following indicators: water permeability in 254 nm, pH, redox potential (ORP), temperaatura (installation at ZUW Goczałkowice - Goczałkowice Water Treatment Plant - Jeziorna 5, 43-230 Goczałkowice-Zdroj, SZW Mikołow - Mikoł-w Branch Operation Department Filaretow 1, 43-190 Mikoł-w, SZW Murcki - Mikoł-w Network Operation Department Murcki Object Białobrzeska Street, 40-749 Katowice, SZW Czarny Las - Black Forest Network Operation Department, Hallera Street 80, 41-709 Ruda Śląska),

d. 1 StResearch stage allowing for the control of water quality in the on-line mode of the following indicators: electrical conductivity, total iron, manganese, general hardness (installation at SUW Łazy - Łazy Water Treatment Station at Wypaleniska 18, 42-525 Dąbrowa Gornicza),

2. provide technical assistance consisting in providing methodical support services, technical support, maintenance services during the periodmaking guarantees and warranties,

3. provide a guarantee and warranty for a period of at least 24 months from the date of signing the fault-free protocol of final acceptance of the order.

The Ordering Party does not set any conditions in this respect.

Contractors who meet the conditions for participation in the proceedings can apply for the award of a public contract and are not subject to exclusion from participation in the procedure.

The Employer will exclude from the Contractor's proceedings in cases, referred to in art. 24 section 1 item 12/13 lit. a / c, item 14/23 of the Act (grounds for exclusionia obligatory). The Orderer will also exclude the Contractor in the following cases - the optional exclusion clauses selected by the Employer, as provided for in art. 24 section 5 of the Act - in relation to which the liquidation has been opened, in the system approved by the court in the restructuring proceedings, the creditors' satisfaction is foreseen by the liquidation of their property or the court ordered a liquorhis estate pursuant to art.332 section 1 of the Act of 15.5.2015 - Restructuring Law, with the exception of the Contractor, who after the declaration of bankruptcy concluded an agreement approved by a final court decision, if the arrangement does not provide for the satisfaction of creditors by liquidating the assets of the bankrupt, unless that the court ordered the liquidation of its property pursuant to art. 366 par. 1 of the Act of 28.2.2003 - Prain bankruptcy (optional transmissions).

The Contracting Authority may, at any stage of the proceedings, summon all Contractors, pursuant to art. 26 par. 2f of the Act to submit declarations and documents (valid as at the date of submission) referred to below. * Prior to the order being awarded, the contractor will call the Contractor whose tender will be assessed at the highest level to submit in the designatedtime (not shorter than 10 days) of the following declarations and documents - 1. Information from the National Criminal Register in the scope specified in art. 24 section 1 point 13 letters a, c, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of bids.

2. A copy from the relevant register or from central records and business information, if separate regulations require an entry-to the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 section 5 point 1 of the Act

3. Declaration of the Contractor about the lack of issuing a final court judgment or final administrative decision on default of payment of taxes, fees or social security or health insurance premiums or in the event of such a verdict or decision of documents afterclaiming payment of these claims together with any interest or fines or a binding agreement on these claims. Declaration of the Contractor about the lack of a decision against him as a preventive measure to prohibit applying for a public order.

5. Contractor in order to demonstrate the lack of grounds for exclusion, referred to in art. 24 section 23 of the Act on time3 days from the date of the call by the Ordering Party, it shall provide the Ordering Party with a declaration of belonging or absence of belonging to the same capital group. With the submission of a statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the proceedings. If the Contractor is established or resides outside the territory of the Republic of PolandPoland instead of the documents referred to:

a) in point 1 above, he shall submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence or the place of residence has a person who is concerned with information or a document, to the extent specified in art. 24 section 1 point 13 letters a, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of bids,

b) in item 2 above, he lodges a document or documents issued in the country in which the Contractor has a registered office or place residence, confirming that its liquidation has not been opened or bankruptcy announced, issued not earlier than 6 months before the deadline for submission of tenders.The contractor applying for the order must show that he has funds on the bank account or cooperative savings and credit union or creditworthiness in the amount of at least PLN 1, 000, 000.00. - The Contractor, in accordance with art. 22a of the Act may, in order to confirm compliance with the conditions for participation in the proceedings specified by the Employer, where applicable, andin relation to a specific order, or its part, rely on technical or professional ability or financial or economic situation of other entities, regardless of the legal nature of its legal relationship.

Contractor, which relies on the ability or situation of other entities, must prove to the Ordering Party that when completing the order, it will have the necessary onesresources of these entities, in particular by presenting the obligation of these entities to provide them with the necessary resources for the needs of the order.

From the document (eg commitment), referred to in point 2 above must result in particular:

a) range of available resources contractor other subject,

b) manner of using the resources of another entity, by the Contractor, in the performance of(c) scope and period of participation of another entity when executing the order, d) whether the entity, on the capacities of which the Contractor relies in respect of the conditions for participation in the proceeding regarding experience, will carry out supplies whose indicated capacities relate to - 1. The application for admission to participate in the proceedings (hereinafter referred to as the application) should be made on the form (or according to such a petition)my scheme) constituting Annex No. 2, which can be found on the website of the Ordering Party in the tab of announcements about orders.

1.1. The Contractor should submit the application together with other documents and statements in the envelope described and addressed in the following manner:

Name and address of the Contractor.

The application must be made in written in Polish, under pain of nullity (the Employer does not agree to submit the application by fax lor in electronic form). The application must be signed by the person authorized to represent the contractor.

The JEDZ statement should be submitted in an electronic form with a qualified electronic signature.

1.2. The Contractor to the application shall attach: - For the preliminary confirmation of the lack of grounds for exclusion and fulfillment of the conditions for participation in the procedure specified by the Employer inSection III of the contract notice valid as of the date of submitting applications, a declaration of non-subject to exclusion and compliance with the conditions for participation in the proceedings submitted in the form of a Single European Order Document (EED), in accordance with the formula set out in Commission Implementing Regulation (EU) 2016/7 of 5.1.2016 laying down the standard form for the Single European DockThe Ordering Party signs the EQUIP statement by a qualified electronic signature issued by the qualified trust service provider being an entity providing certification services, electronic signature, meeting the security requirements set out in the Act of 5/09/2016. for servicech trust and electronic identification (OJ of 2016, item 1579, as amended) in accordance with the Annex in electronic form - a separate file, and Annex No. 3 - Instructions for preparing the EAT by the contractor, which can be found on the website website of the Ordering Party in the tab of announcements about orders.

1.3. Power of attorney established to represent the Contractor / s of the applicantsfor the award of a public contract. The power of attorney should be attached in the original or a notarized copy.

1.4. Document (eg Commitment) of other entities, if the Contractor, in order to confirm compliance with the conditions of participation in the proceedings, relies on the technical, professional or financial or economic capacity of other entities under the principles set out in Art.-22a of the Act

2. The Contracting Authority informs that according to art. 60c section 1 point 1) -4) of the Act:

2.1. The description of the needs and requirements of the Employer is attached as Annex 1 to the contract notice and can be found on the Employer's website in the tender information tab.

2.2. The Awarding Entity does not provide a prize for Contractors

2.3. The preliminary schedule of proceedings is as follows:

a) transmissionthe announcement of the procurement of the Publications Office of the European Union: 12/10/2018

b) Submission by the Economic Operators of requests to participate in the proceedings: 13/11/2018

c) invitation to competitive dialogue: 3-7.12.2018

d ) conducting a dialogue with the Contractors: 17-21.12.2018. e) Inviting the Contractors to submit bids containing the price: 14-18.1.2019 r. f) submitting offers: 4-8.2.2019

2.4. The contracting authority does not provide for the division of the dialogue into stages.

3. The period of being bound by the offer is 60 days with reference to Section IV.2.6 of the procurement notice. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires
