1. The subject of the order is: "Conservation of urban green areas in Zabrze in 2020 2021 ".

Conservation of urban green areas in Zabrze in 2020 2021 in the districts: Helenka, Rokitnica.

A detailed description of the subject of the contract is included in:

- Standards for establishing, maintaining and maintaining urban green areas, Annex 9 to the Terms of Reference,

- List of areas (location of green areas and parks), Annex 10 to the Terms of Reference,

- Bill of quantities for the preparation of the calculationenowa, Annex 13 to the ToR,

- Schedule of works and invoicing, Annex 12 to the ToR.

Pursuant to art. 29 paragraph 3a of the Public Procurement Law, the contracting authority requires employment by a contractor or subcontractor based on an employment contract in the manner specified in art. 22 - 1 of the Act of 26 June 1974, Labor Code (Journal of Laws of 2019, item 1040, as amended) after min. 5 persons planned to carry out tasks foreach of the tasks 1 7.

The contractor must demonstrate conduct of activities in the field of waste management.

In order to demonstrate compliance with the condition regarding competence or authorization to conduct specific professional activities, the contractor will provide possession of an entry in the register referred to in Article 49 paragraph 1 point 4 of the Waste Act of December 14, 2012 (Journal of Laws of 2019, item 701, as amended).

Zamthe aviating entity does not specify the condition.

In order to demonstrate the lack of grounds for exclusion from the contract award procedure and to meet the conditions for participation in the procedure, specified by the contracting authority in point V.2. and V.5. SIWZ should be accompanied by a statement valid as at the date of submitting the bids, in the form of a uniform European order document. In the case of joint application forcontract by contractors, a single European document is submitted by each of the contractors jointly applying for the contract.

In order to confirm that there are no grounds for excluding the contractor from participation in the procedure, the contractor will provide the following documents:

A) information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the PPL issued not earlier than 6 months beforethe deadline for submitting bids;

B) an excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate provisions require an entry in the register or the register, in order to demonstrate the lack of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the Public Procurement Law;

C) the contractor`s statement that he has not issued a final court judgment or final administrative decisionon arrears in the payment of taxes, fees or social security or health insurance contributions or in the event of the issuance of such a judgment or a decision confirming the payment of such receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of these receivables;

D) statement of the contractor that he was not judged against him by avgpreventive prohibition to apply for public contracts;

E) the contractor`s statement of belonging or not belonging to the same capital group in the case of belonging to the same capital group, the contractor may submit together with the statement documents or information confirming that links with another contractor are not lead to distortion of competition in the proceedings.

In prin the case of contractors jointly submitting an offer, documents referred to in point V.6. and V.7. SIWZ is obliged to submit each of the contractors jointly submitting the offer.

In addition, to confirm the lack of grounds for excluding the contractor from the procedure, the contractor within 3 days from the date of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, i.e. information fromopening offers, will provide the contracting authority with a declaration of belonging or non-belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the PPL Act. Along with the submission of the declaration, the contractor may provide evidence that the links with another contractor do not lead to a distortion of competition in the contract award procedure.wcow jointly applying for the award of the contracting authority may request prior to the conclusion of the contract to submit a contract governing the cooperation of these contractors. Such an agreement should specify the parties to the agreement, the purpose of the action, the manner of cooperation, the scope of work envisaged to be carried out for each of them, joint and several liability for the performance of the contract, indication of the duration of the consortium (includingthe implementation of the subject of the contract, warranty and warranty), exclusion of the possibility of termination of the consortium agreement by any of its members until the contract is completed. Before signing the contract, the partners conducting business activity in the form of a civil law partnership shall submit to the contracting authority the contract of partnership.
