1. The subject of the order is: successive delivery of drugs, milk and preparations for enteral nutrition for Prudnicki Medical Center S.A. In Prudnik. The subject matter of the Order consists of 17 parts. 2. The detailed description of the subjectMentioned in the Annexes to SIWZ: No. 1, 1/1 to 1/17 and in the Draft Agreement Annex No. 5 to SIWZ. The condition of participation in the proceedings is the payment of a security in the amount specified for each part of the order in the table below in PLN:

2. The bid bond must be brought before expiry-The deadline for submitting tenders referred to in Chapter XI (1
3). Payment of the bid bond is made by bank transfer to the bank account of the Employer Bank Społdzielczy in Prudnik 69 8905 0000 3000 0021 0887 0001.
4. The bid bond is deemed to be received when it is received on the indicated account of the ordering party. If the deposit is made in money, by bank transfer, the Contractor attaches to the offer of a photocopyCopies of the deposit payment with confirmation of the transfer. On the order of the transfer should be written:

The bid security may be submitted in the forms specified in Article 45 of the Public Procurement Law. / P p In cases where a bid bond is submitted in the forms referred to in point 6, -An offer, confirmed as a true copy of the document constituting a security deposit. The original of this document should be submitted together with the offer without binding it with the offer. Documents referred to in point 9.4. Must remain valid for the duration of the contract, binding the Contractor with the offer. Deposit secured in the forms specified in art. 45 sec. 2-5 of the PPL Act, must include a commitment of the plaintiffA guarantor or guarantor for the occurrence of the events referred to in art. 46 sec. 4a and 5 of the Public Procurement Law Act, where: 1) these documents will contain the terms of payment of the bid bond to the Purchaser unconditionally and on the first request, 2) these documents will be submitted in the original. The ordering party returns the bid bond in the cases specified in art. 46 PPL Law
10. In the case of a mergerBoundary of the bid bond in the form other than in money, the Contractor is obliged to attach to the offer document confirming the establishment of a surety or guarantee in the original. The contracting entity shall keep the bid bond together with interest if: 1) the contractor has been refused the contract for public procurement under the conditions set out in the tender;(4) The Contractor, in response to the summons referred to in Article 4, has failed to make the required performance bond (if required); 3) Conclusion of a public procurement contract has become impossible for reasons attributable to the Contractor; Section 26 (3) and (3a) of the Public Procurement Law, for reasons attributable to him, did not submit documents or statements referred to in art. 25 sec. 1, the statements referred to in art. 25a sec. 1, the plenipotentiary, or did not agree to rectify the mistake referred to in art. 87 section 2 pts. 3 of the Public Procurement Law, which led to the inability to choose the bid submitted by the contractor as the most advantageous.
