Delivery of medicines for treatment of solid malignancies for the needs of the contracting authority under 112 separate positions, detailed in the technical specifications of the OP documentation.

Participants must be allowed to wholesale medicinal products under the terms and conditions of Law on Medicinal Products in Human Medicine (LPPHM), to have the right to carry out activities with drugs and their preparations under the Drug Control ActSubstances and Precursors (LCNSP) - in case the participant offers products that fall within the scope of the law (where applicable) and have the right to transport ionizing radiation sources under the Safe Use of Nuclear Energy Act (ASUNE) - if the participant offers products that fall within the scope of the law (where applicable).These conditions are evidenced when submitting a tender with completion of Section A, Part IV of the EDES, and when signing the OP contract, it is proved with the following documents:

1. Concerning the right for wholesale trade in medicinal products under the LPPHM - it shall be proved by a valid permit and / or a certificate for wholesale trade in medicinal products under Art. 195, para. 1 and para. 2 of the LMPHM and / or manufacturing authorization inthe cases under Art. 196, para 1 of LPPM, and / or import permit in the cases of art. 196, para. 2 of the LMPHM, issued under the terms and conditions of this law.

2. Concerning the right for activities under the LCNSP - it is proved by a valid license under Art. 32, para. 1 of the Law on Control over Narcotic Substances and Precursors for Performing Activities in Narcotic Drugs, Issued under the Terms and Conditionsand this law.

3. Concerning the right to transport sources of ionizing radiation under the ASUNE - shall be proved by a valid license for carriage of radioactive substances issued under the terms and conditions of the ASUNE - Art. 58, para. 1, item 4 in connection with Art. 56, para. 1 from s.z.

Non-resident participants should be entered in similar registers of the points mentioned in the previous points according to the legislation of the countrythe Member State in which they are registered, and at the conclusion of the contract they must submit the documents under items 1 - item 2. 3.

According to Article 67, paragraph 4 of the Public Procurement Act, the EDES is submitted in electronic form, signed with electronic signature.
