1. The subject of the order is: delivery of medicinal products -78 tasks, the detailed description and scope of which are included in Annex 1-78 to the ToR.

Ceftolozanum Tazobactamum 1g 0.5g, powder for concentrate for solution for infusion, op. 10 vials

The condition will be fulfilled by a Contractor who has the authorizationto conduct specific activities or activities which are the subject of this proceeding, i.e. having the Permission of the Main Pharmaceutical Inspector to operate a Pharmaceutical Warehouse or other equivalent document. (EU) 2016/7 of 5 January 2016 NOTE! EAT must be sent together with the offer in the form of an electronic document with a qualified electronic signature (in accordance with the rules for submitting the offer described in part XI of the Terms of Reference)

Information from the National Criminal Register in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act

Certificates of the competent head of the tax office confirming thatthe contractor is not in arrears with payment of taxes, the certificate of the relevant organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, an excerpt from the relevant register or from central records and information on business activity, e. declaring the contractor not to adjudicate against him as a precautionary measure prohibiting him from applying for public procurement;

Contractor`s declarations of not issuing a final court judgment or a final administrative decision to him regarding late payment of taxes, fees or social or health insurance contributions or in the event of such a judgment or decision bydocuments confirming the payment of these debts together with any interest or fines or conclusion of a binding agreement on the repayment of these debts;

Declaration of the contractor not to pay taxes and local fees, referred to in the Act of 12 January 1991 on local taxes and fees (Journal of Laws U. of 2016, item 716); Permits of the Main Inspector of Farmsfor the maintenance of the Pharmaceutical Warehouse or other substantive documentation;
