Successive supplies of pharmaceutical products. The subject of the order consists of 6 indivisible tasks. Task No. 1. Intravenous trastuzumab; Task No. 2. Trastuzumab subcutaneous; Task No. 3. Lapatinib; Task No. 4. Dexamethasone phosphate; Task No. 5. Sofosbuvir, Velpatasvir; Task No. 6. Glecaprevir, Pibrentasvir, Rybawiryna.

Contractors who meet the conditions for participation in the procedure, ie they have a license, license or license for wholesale trade in medicinal products, may apply for the order,

In order to confirm the lack of grounds for exclusion, the Contractor shall submit the following documents - a) certificate of the competent chiefof the tax office confirming that the contractor is not in arrears with paying taxes not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the contractor has concluded an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained the legally required exemption, deferred(b) a certificate from the relevant local organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with the payment of social security or health insurance premiums, issued no earlier than3 months before the deadline for submission of tenders, or other document confirming that the contractor has entered into an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally required exemption, postponement or installment of overdue payments or suspension in installments the entire implementation of the decision of the competent authority, - c) declarationthe contractor on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2017, item 1785, consolidated text with later amendments),

d ) information from the National Criminal Record in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act issued not earlier than 6 months before the deadline for submitting bids,

e) Ifthe contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in items 4a, b, d - submit: • information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body the country in which the contractor has its registered office or place of residence or place of residencethe person concerned by information or a document within the scope specified in art. 24 sec. 1 point 13, 14 and 21 - information, document or documents should be issued not earlier than 6 months before the deadline for submission of tenders, • document or documents issued in the country in which the contractor has its registered office or place of residence, confirming that: it is not in arrears-taxes, fees, social security or health insurance contributions or that it has entered into an agreement with the competent authority regarding the repayment of such receivables with any interest or fines, in particular it has obtained the legally required exemption, deferral or payment of overdue payments or full suspension of the implementation of the decision competent authority - the document or documents should-be issued not earlier than 3 months before the deadline for submitting bids,

f) If at the place of residence of the person or in the country in which the contractor has its registered office or place of residence, the documents referred to in point 4e are not replaced by a document containing the contractor's statement, indicating the person or persons authorized to represent it or statementsie the person whose document was to be submitted to a notary or before a judicial, administrative or professional self-government or economic authority competent for the seat or place of residence of the contractor or place of residence of that person. Documents should be issued appropriately on the dates specified in point 4e

The contracting party does not set conditions for participation in the proceedings in this respect.

The contractor is obliged to submit the bid bond before the deadline for submission of tenders in the amounts specified below for the individual tasks: - Task no. PLN 1 - PLN 6, 000.00 - Task No. 2 - PLN 6, 000.00 - Task No. 3 - PLN 2, 000.00 - Task No. 4 - 500.00 PLN

Task No. 5 - PLN 50, 000.00 - Task No. 6 - PLN 200, 000.00

List of required statements and documents

1. List of statements and documents submitted together with the offer valid as at the day of submitting bids: a) a statement confirming the absence of grounds for exclusion and fulfillment of the conditions for participation in the proceedings in the form of a completed single document (on the electronic version) (from-except for parts deleted by the Ordering Party),

b) Form filled in and signed, Appendix No. 1 to the ToR, - c) completed and signed Price Form - Annex 2 to the ToR,

d) proof of payment of the bid security, - e) power of attorney to sign the offer - if the offer is signed by a person / persons acting on its basis,

f) a document establishing a proxy in the case of the submission oferty share (Article 23 of the Act),

g) a statement regarding the data set out in point P.1 SIWZ,

h) statements for the preliminary confirmation of the lack of grounds for exclusion and fulfillment of the conditions for participation in the proceedings in the form of a single document - each contractor in the case of joint application, - and) statements for the preliminary confirmation of the lack of grounds for exclusion and meetconditions for participating in the proceedings in the form of a single document - concerning entities for resources cited by the contractor in order to demonstrate the absence of grounds for exclusion against them and fulfillment, in so far as it refers to their resources, the condition of participation in the proceedings. List of the required declarations and documents valid as at the date of submission to confirm compliance withshare prices in the proceedings, which the contractor submits in the proceedings on the ordering party's request: - - a license, license or license for wholesale trade in medicinal products.

3. List of the required declarations and documents valid as at the day of submission to confirm that the deliveries, services or works fulfilled by the requirements set by the contracting authority, which the contractor submits todulling the ordering party's request:

• The Employer does not request any documents in the above-mentioned scope4. Contractor's declaration of belonging or not belonging to the same capital group within the meaning of the Act of 16.2.2007 on competition and consumer protection (Journal of Laws of 2017, item 229, consolidated text of later amendments) with other contractors they submitted offers in the proceedings--contractor within 3 days from the date of posting on the website the information referred to in Article 86 par. 5 of the Act, it provides the orderer with a declaration of belonging or non-affiliation to the same capital group within the meaning of the Act of 16.2.2007 on competition and consumer protection with other contractors who submitted bids in the proceedings or prove that existing meattheir links do not lead to distortion of competition in the procurement procedure. Information regarding the submission of bids by contractors jointly applying for the award of a contract (Article 23 of the Act). In the event that the contractors jointly apply for the order:

a) appoint a proxy to represent them in the proceedings for granting this order or to a repr(b) together with the offer each contractor must submit a document referred to in point D.1.h) - this document is confirmed by the lack of grounds for exclusion and fulfillment of the conditions for participation in the proceedings in the extent to which each of the contractors demonstrates fulfillment of the conditions for participation in the proceedings and no grounds for exclusion, c) each of thethe voters forward the statement referred to in point D.5 on the terms specified therein, d) each of them at the request of the ordering party must submit statements and documents referred to in point D.4.a - f, e) the transmission of declarations and documents during the proceeding will be made only from proxy (leader).
