The subject of the order is the provision of the service of delivery and collection of postal parcels and courier parcels at the location Gdańsk, Gdynia, Sopot, Rumia Reda, Wejherowo, Tczew, Pruszcz Gdański in accordance with the Description of the subject of the order constituting Annex 5 to the ToR and the specimen agreement constituting Annex 3 to the ToR. The subject of the order was divided into 10 Parts.

Main Site: Provision of delivery and collection of postal parcels and courier services in Gdańsk 2 according to OPZ.

Maximum number of services during the periodand contracts 517 776 items

1. Contractors, who are not subject to exclusion pursuant to art. 24 sec. 1 and art. 24 sec. 5 point 1 of the Public Procurement Law. In order to demonstrate the absence of grounds for exclusion, the Contractor shall be required to submit the following declarations and documents: a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13) lit.a) c), art. 24 sec. 1 point 14) in conjunction with from paragraph 1 point 13) lit. a) c) and art. 24 sec. 1 point 21) of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,

b) excerpt from the relevant register or from the Central Register and Information on Economic Activities, if separate regulations require entry in the register or records to confirm the lack grounds for exclusion nand based on art. 24 sec. 5 point 1 of the Public Procurement Law Act,

c) the Contractor's statement that he has not issued a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums or in the event of such a verdict or decision confirming the payment of these payments receivables with possible onesinterest or fines or conclusion of a binding agreement regarding repayment of such receivables, d) declaration of the Contractor about the lack of a ruling against him as a precautionary measure prohibiting applying for public orders, - e) within 3 days from the date of posting on the Platform information about which in art. 86 par. 5 of the Public Procurement Law, shall submit to the Ordering Party via the Platform a declaration on pbility or lack of belonging to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law (prepared using the template constituting Annex No. 3 to the Terms of Reference). With the submission of a statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure. In the case of a joint applicationthe Contractor shall submit the above-mentioned certificate by each Contractor,

f) a statement valid as at the date of the submission of bids, constituting preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The Contractor submits this declaration in the form of a Single European Order Document (EIN) to the extent specified by the Employerin SIWZ in electronic form and provides a qualified electronic signature.

2. The Contractors who meet the conditions for participation in the procedure may apply for the award of the contract: The Employer does not specify the terms of competences or entitlements to conduct a specific professional activity, economic or financial situation and technical or professional capacity. In progressThe contractor is not subject to exclusion in the case referred to in art. 24 sec. 1 point 13 lit. d) of the Public Procurement Law, and in the case referred to in art. 24 sec. 1 point 14 of the Public Procurement Law, if the person referred to in this provision has been convicted of an offense listed in art. 24 sec. 1 point 13 lit. d) Public Procurement Laws

1. If the Contractor is established or residesoutside the territory of the Republic of Poland, instead of the documents referred to in Chapter III.1.1) of this notice: - 1) lit. a) SIWZ - shall submit information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence or place of residencea person who is affected by information or a document, to the extent specified in art. 24 sec. 1 point 13 lit. a-c), art. 24 sec. 1 point 14) in conjunction with from paragraph 1 point 13) lit. a) - c) and art. 24 sec. 1 point 21) of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

2) lit. b) submit a document or documents issued in the country in which the Contractor has his registered office or place of residence, confirming, respectively, that its liquidation has not been opened or that bankruptcy has been declared. The documents referred to in point 1 item 1 and item 2 of this chapter should be issued not earlier than 6 months before the deadline for submission of tenders. If in the country in which the Contractor has a registered office or place of residence or a place of residence has the person the document refers to, do not issuee of the documents referred to in point 1 of this chapter are replaced by a document containing the Contractor's declaration, indicating the person or persons authorized to represent it, or a statement of the person to whom the document concerned, submitted to a notary or before a judicial, administrative or administrative body or an organ of professional or economic self-government competent forto the seat or place of residence of the contractor or place of residence of that person. The provision of point 2 applies.

4. The document referred to in point III.1.1), lit. f) this announcement shall be submitted in electronic form and shall be provided with a qualified electronic signature.

5. The document referred to in point III.1.1), lit. e) of this announcement, shall be submitted within 3 days from the date of placement onThe information platform referred to in art. 86 par. 5 Pzp.

6. The contracting party, prior to the order being awarded, will call the Contractor whose tender has been best assessed, to submit within the prescribed period, not shorter than 10 days, the current date as of the day of submitting declarations or documents referred to in point III.1.1). a) III.1.1) lit. d) of this announcement. Note: The Contractor may limit himselfto fill only section α of part IV of the EAT form (general declaration of compliance with the conditions for participation in the procedure). Appropriate (evidence-based) verification of the fulfillment of specific conditions of participation in the procedure (selection criteria), the awarding entity will only perform in relation to the contractor whose bid was rated the highest (in accordance with point II.2.5) of this announcement).
