Delivery of postal parcels and courier parcels - B

The subject of the order is the delivery of postal parcels and courier shipments as part of the service B. The service will be provided on the territory of the cities located within the Lodz voivodship. The estimated number of parcels to be served during the duration of the contract is equal to-1 671 500 pieces.

Main Site: Administrative area of Aleksandr-w Łodzki.

The subject matter of the order is delivery of postal parcels and courier services as part of the B service, within the meaning of the Postal Law and the Regulations on the Pocztex service in the domestic trade, hereinafter collectively referred to as "parcels", as part of which the Contractor undertakes to provide the Employer with the task of relyingo on: a) collection of postal parcels and courier shipments within the meaning of art. 3 points 14 and 19 of the Act of November 23, 2012. Postal Law and Regulations for the provision of Pocztex in domestic services (hereinafter referred to as "parcels") from the place indicated;

b) moving packages,

c ) delivery of parcels at the address indicated on the package, in the case of parcels sent with the option "Collection at point" transferhandling parcels to the collection point being a post office or to a partner point, or at the pick-up point,

d) telephone contact with the addressee to determine the date and place of delivery;

e) delivering courier mail "Pocztex Procedure" in accordance with the activities described in the Checklist; - f) acceptance from the recipients of fees and charges incumbent on the packagewhich are not paid by the senders and settlement of all collected receivables;

g) return in the indicated place of undelivered parcels and charges and receivables accepted from the recipients on parcels that have not been paid by the sender;

h) accepting parcels from senders, the parcel accepted from the sender is considered to be a parcel accepted from the indicated sender oonce delivered to an indicated post office or actually delivered, - i) performing factual activities within the meaning of art. 6a paragraph 1 point 2 of the Act of 29.8.1997 Banking Law (Journal of Laws of 2017, item 1876), consisting in verification of the identity of a person concluding a contract with a bank in the manner specified in the Checklist; - j) performing other activities related to implementation of services to• ordered number of parcels to be served during the contract: 85, 500 pcs.

Contractors who are not subject to exclusion under Article 24 sec. 1 and art. 24 sec. 5 point 1 of the Public Procurement Law - 1. In order to demonstrate the absence of grounds for exclusion, the Contractor shall be required to submit the following declarations and documents:

a) informafrom the National Criminal Register to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,

b) excerpt from the relevant register or from central records and business information, if separate regulations require entry in the register or records in order to confirmation of the lack of grounds for exclusion based onArt. 24 sec. 5 point 1 of the Public Procurement Law Act,

c) the Contractor's statement that he has not issued a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums or in the event of such a verdict or decision confirming the payment of these payments receivables with possible interest or lub) fines or conclusion of a binding agreement regarding the repayment of these debts, d) declaration of the Contractor about the lack of a ruling against him as a preventive measure of the prohibition to apply for public orders, - e) statements of affiliation or non-affiliation to the same capital group referred to wart.24 par. 1 point 23 of the Public Procurement Law. Along with the submission of the statement, the contractor may pto provide evidence that links with another Contractor do not lead to distortion of competition in the proceedings, f) current statement as a Uniform European Order Document prepared as in the standard form defined in the European Commission Implementing Regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU and art.80 ust. 3 of Directive 2014/25 / EU (hereinafter referred to as FOOD) submitted in electronic form with a qualified electronic signature. The information contained in the statement will constitute preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings. The scope of the statement is indicated by the Ordering Party in the xml format on the Employer's website dthe number of proceedings PI.TPZKr.2600.077.2018.

1. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point III.1.1) 1 of this announcement:

- lit. (a) submit information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative bodyparadise in which the Contractor has a registered office or place of residence or place of residence has a person who is concerned with information or a document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law,

- lit. b) makes a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming, respectively, that its liquidation has not been opened or announced-bankruptcy.

2. Documents referred to in point 1 (a) and (c) b) of this chapter, should be issued not earlier than 6 months before the deadline for submission of tenders

3. If in the country in which the Contractor has his registered office or place of residence or place of residence has the person concerned, the documents referred to in point 1 of this chapter shall not be replaced byit is a document containing the Contractor's declaration, indicating the person or persons authorized to represent it, or a statement of the person to whom the document was to be applied, submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the contractor's domicile or place of residence or place of residence ta person. The provision of point 2 applies.

4. The document referred to in point III.1.1) 1 lit. f) this announcement shall only be submitted in electronic form bearing a qualified electronic signature in the manner specified in the ToR. The electronic communication means used for submitting EAT by the Contractor is e-mail. EAT should be sent to the following e-mail address: pi.zakupy.krakow@poczta-polska.pl. The signed EID electronic document should be encrypted, i.e. with an access password. The ordering party requires encryption using the 7-zip application, the AES-256 encryption method, and a minimum password of 12 characters. The Contractor places the password of access to the EED file in the content of his offer.

5. Document referred to in point III.1.1) 1 point e) of this announcement, shall be submitted within 3 daysfrom the date of posting on the website information referred to in art. 86 par. 5 Pzp.

6. The ordering party will apply the procedure specified in art. 24 aa of the Public Procurement Law. In connection with the above, the Ordering Party, prior to the order being awarded, will call the Contractor whose offer has been best assessed, to submit in the designated day, not shorter than 10 days, the current daydeclarations or documents referred to in point III.1.1) .1. a) - d) of this announcement. Note: The Contractor may confine himself to completing only section α of Part IV of the EED form (general statement of compliance with the conditions for participation in the procedure). A proper (evidence-based) verification of the fulfillment of the conditions for participation in the procedure (selection criteria) will only be made by the Employer-to the Contractor whose bid was rated the highest (according to point II.2.5) of this announcement)
