Disposable tip for harmonic knife - arm length 36 cm or 23 cm, diameter 5 mm, active bracelet made of titanium alloy covered with a black coating that minimizes adhesion. The tip has 3 buttons activating min for the minimum power level, max for the maximum power level. Possibility of cutting and coagulation, pistol grip shape.
Disposableand tip for harmonic knife 9 cm long. The tip has two buttons to activate max and min. A tip with built-in adaptive tissue technology that allows the generator to identify and monitor the instrument during use, which allows the generator to modulate and reduce output power as well as generate user feedback signals as needed. Disposableand the handle of the endoscopic stapler with built-in articulation, designed for loads performing anastomosis 60 mm long, with two closing and trigger levers. Arm length 28 cm, 34 cm, 44 cm.
Disposable linear charges for a simple endoscopic stapler, allowing an anastomosis of 60 mm, loaded into the jaws of the stapler. Loads for thin tissue (height1 mm staples after closing), loads for standard tissue (1.5 mm staple height after closing), intermediate tissue loads (1.8 mm staple height after closing), heavy tissue loads (2 mm height after closing). (The Purchaser will each time specify the size of the load when placing the order.
7.4. Competence or authority to conduct specific professional activities provided thatthis results from separate provisions on confirmation, the Ordering Party will recognize the submission of an authorization for operating a pharmaceutical warehouse in accordance with the Act of 6.9.2001 Pharmaceutical Law (Journal of Laws of 2017 item 2211)
7.5.1. Current excerpt from the relevant register or from the central register and information on business activity, if separate provisions require an entry in the register in order to showtowards the grounds for exclusion based on art. 24 paragraph 5 point 1 of the Act.
7.5.2. Current certificates of the competent head of the Tax Office and the relevant branch of the Social Insurance Institution or the Agricultural Social Insurance Fund confirming respectively that the Contractor is not in arrears with the payment of taxes, fees and health or social insurance contributions or certificates that theyfaeces provided for by law, release, deferment or installment of overdue payments or suspension in full of decisions of the competent authority - issued not earlier than 3 months before the deadline for submission of tenders.
7.5.3. Current information from the National Criminal Register in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6months before the deadline for submitting requests to participate in the contract award procedure or the submission of tenders.
8. Submission of bids by jointly operating entities
8.1. If a joint bid is submitted by several entities, each of them is required to submit the documents issued for it listed in points: 7.5.2-7.5.4 in JEDZ (part III EAT), whileentities submit joint documents, i.e. the conditions specified therein are met when the entities submitting the offer meet them jointly.
8.2. A joint bid must be prepared and submitted as follows:
8.2.1. jointly submitting entities establish and indicate a representative to represent them in the proceedings for the award of this contract or to represent them in the course ofrequest for the award of this contract and conclusion of a public procurement contract - a power of attorney should be attached to the offer;
8.2.2. the offer must be signed in such a way as to legally oblige all entities jointly submitting the offer;
8.2.3. each of the entities submitting a joint bid must submit a statement that it is not subject to exclusion from the procedure pursuant to art. 24 ust. 1 of the Act - and must submit the documents related thereto, listed in points 7.5.2-7.5.4.
8.3. All correspondence will be conducted only with the proxy.
8.4. Contractors jointly applying for this contract, whose tender will be considered the most advantageous, before signing the contract are obliged to provide the Employer with an appropriate regulatory contractcooperation of these contractors.
9. Contractors having their registered office or place of residence outside the territory of the Republic of Poland
9.1. If the contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in item 7:
9.1.1. point 4 - submits information from the relevant register or, in the absence of such register, another equivalent issue documenty by the competent judicial or administrative authority of the country in which the contractor has its registered office or place of residence or place of residence, the person to whom the Information or document relates, to the extent specified in art. 24 paragraph 1 point 13. 14 and 21 and para. 5 points 5 and 6 of the Public Procurement Law;
9.1.2. points 5.2-5.4 - submits a document or documents issued in the country in which the contractor has its registered office or place of residencekite, confirming respectively that:
9.1.2.1. he is not in arrears with payment of taxes, fees, social security or health insurance contributions.
Requirements for a security deposit
13.1. Based on Article. 45 item 1 of the Act, the Awarding Entity requests a bid security, which is for individual packages:

13.1.1. The bid bond should be brought before the expiry of terminu make offers. The date of payment of the bid security is the day on which the Purchaser's account is credited by 11:00 am. The bid bond must cover the offer validity period.
13.1.2. The bid bond may be lodged in one or several forms mentioned in art. 45 item 6 of the Act, i.e. :
13.1.3. in cash (by transfer to the Employer's account: Bank PKO BP S.A. o / Polanica-Zdroj 02 1020 3668 0000 5202 0476 2730,
13.1.4. in handrailsbank or guarantees of the cooperative savings and credit union, provided that guaranteeing the cash is always a monetary guarantee,
13.1.5. in bank guarantees,
13.1.6. in insurance guarantees,
13.1.7. in sureties granted by entities referred to in art. 6b paragraph 5 point 2 of the Act of November 9, 2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2018 item 110, 650, 1000, 1669).
Proof of payment of the bid security must be attached to the offer in electronic form.
13.2. Deposit refund
13.3.1. Employer, in accordance with art. 46 section 1 of the Act returns the bid bond to all Contractors immediately after the selection of the most advantageous offer or annulment of the procedure, with the exception of the Contractor whose offer has been selected as the most advantageous and subject to point16.5.
13.3.2. Employer, in accordance with art. 46 section 3 of the Act, requires the bidder to be lodged again by the Contractor who has been refunded the bid security pursuant to Art. 46 section 1 of the Act, if as a result of deciding the appeal his offer was chosen as the most advantageous. The Contractor shall lodge a deposit within the period specified by the Employer.
13.3.3. If the bid bond was paid in cash, the Purchaser shall refund it forbank account indicated by the Contractor.
13.4. Retention of the bid security
13.4.1. The Employer will keep the bid bond if the Contractor in response to the call referred to in art. 26 section 3 of the Act, did not submit documents or statements referred to in art. 25 paragraph 1 of the Act or powers of attorney, unless he proves that this is due to reasons not attributable to him.a whose offer has been selected:
13.4.1.1. refused to sign a public procurement contract on the terms set out in the offer,
13.4.1.2. did not provide the required security on due performance of the contract, if required by the Employer,
13.4.1.3. conclusion of the public procurement contract became impossible for reasons attributable to the Contractor.
10. Confirmation Documentsthat the offered deliveries meet the requirements specified by the Employer (subject documents submitted in electronic form with a qualified electronic signature).
To confirm the requirements, the Contractor together with the offer shall submit for the assortment listed in Annex 1 of the ToR:
10.1. Current certificate of admission to trading and use in accordance with the Act of 20.5.2010on medical devices (i.e., Journal of Laws of 2017, item 211, as amended), and the Pharmaceutical Law Act of 6.9.2001 (Journal of Laws of 2017, item 2111 as amended as amended),
10.3 CE declaration of conformity issued by the manufacturer and the certificate of the notified body in the area of medical devices - required by law or notification to the register of medical devices of a device marked with the CE mark, for whicha declaration of compliance was filed.
