Supply of masks for filtering solid and liquid particles for the Branches of Polska Grupa Gornicza S.A. according to the assortment and in the estimated quantities specified in the offer form constituting Annex 2 to the ToR.

If the Contractors obtained net income in one financial year in the last 3 financial years, and if the period of business activity is shorter than one year in this period, on the basis of the "Profit and loss account" item net income from salessupply of products, goods and materials or net revenues from sales and equivalent to them with a value not less than:

Total for the implementation of the entire object of the contract PLN 4 092 000.00. In the case of Contractors not required to prepare financial statements, the Employer will verify the economic situation of the Contractor on the basis of other documents confirming revenues for the period as in the previous sentence. In the case of submitting an offer for more than one part of the order, the value of revenuesu net obtained in one financial year in the last 3 financial years, and if the period of business is shorter than one year in this period, it may not be less than the sum of the values specified for individual parts of the contract for which the Contractor submits the offer. In the case of Contractors who present revenues in foreign currencies, the Employer will convert the listsof the amount according to the average NBP exchange rate announced on the last day of the year to which the revenues relate, and in the case of revenue obtained in the current year according to the average NBP exchange rate announced on the day preceding the day on which the offer submission deadline expires. In the event that the financial year does not coincide with the calendar year, the Employer will convert the indicated amount according to the average NBP exchange rate announcedon the last day of the month of the financial year.
2. The Contractor whose offer will be rated the highest or which the Employer may request pursuant to art. 26 paragraph 2f of the Public Procurement Law, will be obliged to submit an original or a copy certified by the Contractor for compliance with the original of the "Profit and Loss Account" for one financial year from the last 3 financial years, and if the period of business activity isis less than one year for this period. In the case of entities which, pursuant to separate provisions, are not obliged to prepare financial statements, the Employer requires other documents specifying revenues for the period as in the previous sentence.
