Provision of land medical transport

This consultation is launched by the support establishment of the Regional Hospital Group (GHT) Rhone Vercors Vivarais in accordance with the provisions of article R. 6132-16 of the CSP. It concerns all of the party establishments, with the exception of EMS Le Perron. The procurement procedure for this contract is a call for tenders open within the meaning of articles L. 2124-1 and L. 2124-2 of ordinance no 2018-1074 of 26.11.2018 of the public procurement code. As regards a framework service agreement, it is subject to the provisions of article R. 2162-4 of the public procurement code, executed by means of purchase orders issued by each of the establishments.issues concerned (articles R. 2162-13 and R. 2162-14 of the public procurement code), without minimum or maximum, based on the estimated indicative quantities defined in the CCP, not binding establishments. -hospitaliers CH Valence - HDN Romans s / Isere.

a) application via the ESPD:

- dematerialized application form ESPD (including the informationrelating to the form of the application, power of the signatory, turnover, staff of the candidate),

- references of the candidate: executed during the last 3 years indicating the amount, the date, the recipient (public or private),

- technical capacity of the candidate;

b) classic application:

- duly completed application letter form DC1,

- declaration of the candidate form DC2. VSthis declaration, duly completed, in particular section D on turnover must be completed by:

—— copy of the judgment or judgments in the event of receivership,

—— copy of the documents relating to the powers of the person empowered to initiate the candidacy and this up to the operation considered. If grouping copy of the power of attorney - declaration on honor that the candidat is not the subject of a conviction for an offense provided for in article 45 of ordinance n 2015-899 of 23.7.2015.

- candidate`s references: executed during the last 3 years indicating the amount, the date, recipient (public or private),

- technical capacity of the candidate,

- declaration of subcontracting in the event of declaration at market notification,

- risk insurance pprofessional and decennial.

c) documents to be provided under the offer. It is imperative to provide a complete file, fully compliant with the requirements of the regulations and the consultation file: of the grouping,

- annex 3 filled in integraalso:

—— if subcontracting, provide in annex a complete DC4 form, date, sign,

—— if grouping, provide in annex the detailed distribution and the amount corresponding to the services of each member,

- a technical memory setting out the different elements of the offer and the methods of performing the service subject to the contract, organized in separate chapters according to the criteriares and sub-criteria:

- Chap. 1 technical value:

1) organization proposed to ensure the provision of transport;

2) organization proposed to ensure the monitoring of the service;

3) hygiene and cleanliness of vehicles and equipment,

- Chap. 2 sustainable development:

1) social approach;

2) environmental approach.

The deposit of folds must be imperatively delivered by dematerialized way.This consultation benefits from the DUME service.
