The subject of the order is a set of digital radiography for industrial research, which must ensure the production of radiographs of welded joints in test class B according to PN -EN ISO 17636-2, for the Transport Technical Supervision Laboratory in Krakow.

PThe object of the order is a set of digital radiography for industrial research, which must ensure radiograms of welded joints in test class B according to PN-EN ISO 17636-2, for the Transport Technical Supervision Laboratory in Krakow, consisting of: a) DDA 2 detection panel pcs,
b) device for handling the detection panel 2 pcs,
c) program for acquisition, processing andRadiography of rigid DR panels, CR scanners and digitizers 2 licenses,
d) station for viewing, analyzing and archiving digital radiograms 1 pc,
e) monitor 1 pcs,
f) wired module increasing the range of digital detector work 2 pcs .,
g) software - exposure calculator, dedicated for the offered detection panel in conjunction with the indicated X-ray tube, calculating the correct exposure times 2 licenses,
h) industrial X-ray lamp, metal-ceramic, constant-voltage 1 set,
i) duplex double-screw pointer according to EN 19232-5 with D13 certificate - 2 pieces and D15 1 piece including 3 items
2. Contractors, who are not subject to exclusion pursuant to art. 24 sec. 1 Public Procurement Law Acts, also ordering pit will rule out the exclusion of the Contractor based on art. 24 sec. 5 point 1 of the Public Procurement Law - List of statements and documents, which the Contractor submits with the offer:
1) power of attorney, if the rules of representation are not clear from the registration document
2) valid on the day of submitting the initial statements EAT
3) The contractor who relies on the abilities of other entities to confirm the fulfillmentia conditions of participation in the proceedings, he must prove to the Employer that while fulfilling the order, he will have the necessary resources of these entities, in particular by presenting a commitment (declaration) of these entities to give him the necessary resources for the order
3. Each Contractor within 3 days from the date of posting on the website information aboutreferred to in Article 86 par. 5 of the Public Procurement Law, will provide the Employer with a declaration of belonging or non-affiliation to the same capital group
4. List of statements and documents submitted by the Contractor at the request of the Employer
2) to confirm the absence of grounds for exclusion of the Contractor from participation in the proceedings The Contracting Authority requests:
a) information from the National Criminal Register in the scopein art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law and issued not earlier than 6 months before the deadline for submitting bids,
b) excerpt from the relevant register or from central records and business information, if separate regulations require entry in the register or records, in order to demonstrate the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the PZP Act issuedit is not earlier than 6 months before the deadline for submitting bids,
a) the Contractor's statement that he has not issued a final court judgment or final administrative decision on defaulting taxes or social security or health insurance premiums or in the event of such a decision or decision of the documents confirming the payment of these receivablesz with possible interest or fines or conclusion of a binding agreement regarding repayment of such receivables (in accordance with the model constituting Annex 4 to the ToR),
b) declaration of the Contractor about the lack of a ruling against him as a preventive measure of prohibiting applying for a public order (in accordance with constituting Annexe No. 4 to the ToR);
If the Contractor is a registered office or a place ofoutside the territory of the Republic of Poland, instead of the documents referred to in paragraph 4 point 2:
1) lit. a) submit information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence has the person to whom the information relatesa or a document, to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law - the document must be issued not earlier than 6 months before the deadline for submitting offers;
2) lit. b) submits a document or documents issued in the country in which he has his registered office or place of residence, confirming that his liquidation has not been opened, and no bankruptcy has been announced - the document should be displayedy not earlier than 6 months before the deadline for submitting bids,

The ordering party will follow the procedure referred to in art. 24aa paragraph 1 of the Public Procurement Law. The ordering party in the present proceedings will first evaluate the offers, and then will examine whether the Contractor whose offer has been evaluated as the most advantageous, not forega exclusion and meets the conditions for participation in the proceedings.
