The subject of the order is the performance of a maintenance water robot in the area of Water Surveillance in Hrubiesz-w, Tomasz-w Lubelski, Włodawa.

Main Site: Area: gm. Werbkowice, afterw. Hrubieszowski, commune Miączyn, area Zamojski; commune and the city of Biała Podlaska.

The subject of the order is to keep the government. A hut from Horysz-w in km 0 000-10 056.

The scope of the maintenance robot includes:

- Manual mowing of lichens from the slopes and the bottom of the watercourse,

- Raking the mowed vegetation from the bottom of the watercourse,

- Shearing bushes,

- Manual disassembly of beavers there

Ordering he did not specify the condition of participation in progressin this respect.

The Contracting Authority did not specify the condition of participation in the proceedings in this respect.

1. Contractors are subject to exclusion pursuant to art. 24 sec. 1 point 12-23 and Article 24 para. 5, pts. 1 and point 4 of the Public Procurement Law Act.

2. Together with the offer, the contractor submits a statement valid as of the date of the submission of bids constituting preliminary confirmation that the Contractor is not subject to exclusion from progressand meets the conditions for participation in the proceedings. The declaration is required to be submitted in the form of a Single European Order Document (EIN), in electronic form

3. At the request of the ordering party within the prescribed period, not shorter than 10 days, the contractor whose offer has been rated the most, submits the current statements and documents confirming the fulfillment as of the day of submission.by the contractor of the conditions of participation in the proceedings and no grounds for exclusion from the proceedings:

3.1. List of services performed not earlier than in the last three years before the deadline for submission of bids, and if the period of business is shorter - in this period, together with their value, subject, dates of completion and entities have the benefit of which services were made fromcombining evidence that these services have been duly performed, the evidence being referred to are references or other documents issued by the entity for which the services were performed, and if for a justified reason of an objective nature the contractor is unable to obtain these documents - contractor's statement. The scope of references and other documents must confirmcompliance with the condition set out in point 5.1. point. a) SIWZ

3.2. Information from the National Criminal Record in the scope specified in art. 24 sec. 1 point 13 and 14 of the Act, issued not earlier than 6 months before the deadline for submitting offers;

3.3. Information from the National Criminal Record in the scope specified in art. 24 sec. 1 point 21 of the Act, issued not earlier than 6 monthsd the deadline for submitting offers,

3.4. A copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act,

3.5. Declaration of the Contractor about the lack of a decision against him as a preventive measure to prohibit the application of pI am asking for an order to confirm the absence of grounds for exclusion under Art. 24 sec. 1 point 22 of the Act,

3.6. The Contractor's statement that he has not been the subject of a final court decision or final administrative decision on default of taxes, social security or health insurance premiums or social security contributions, or in the event of a verdict or decision being issuedmaking payments of these claims together with possible interest or fines or conclusion of a binding agreement on repayment of such claims.

3.7 If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to: -a) in item 1.3 and 3.3. submit information from the relevant register or, in the absence of such a register, -another equivalent document issued by a competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence has a person who is concerned with information or a document, to the extent specified in art. 24 sec. 1 item 13, 14 and 21 of the Act;

b) point 3.4 shall submit a document or documents issued in the country in which the Contractor has a registered office or place of residencea, confirming that its liquidation has not been opened and that bankruptcy has not been announced. - Documents referred to in sub-items a) and b), should be issued not earlier than 6 months before the deadline for submission of tenders.
