Total weight of the pallet with bags max. 1, 000 kg.

The customer is a taxpayer of the agricultural tax and the ordered product is usedthis for own needs.

I. Contractors who are not subject to exclusion based on the Act may participate in the procurement procedure. Art. 24 paragraphs 1 and 5 of the Public Procurement Law

II. To confirm compliance with the conditions for participation, the Contractor must submit:

1. offer form;

2. together with the offer a statement in the form of a uniform document completed to the extent indicated by the Employer in the stalldeclaring about the order and the Specification of the Essential Terms of the Order:

a) the declaration is submitted in the form of a uniform document, in order to confirm the non-exclusion and fulfillment of the conditions for participation in the proceedings;

b) the Employer submits in the attachment the template of the declaration constituting Annex 2 to the ToR developed on the basis of form established in the Eur Commission Implementing Regulationopejska issued on the basis of art. 59 clause 2 of Directive 2014/24 / EU and art. 80 clause 3 of Directive 2014/25 / EU;

c) the statement should be valid as at the date of submission of tenders;

d) The Contractor may use in the single document still current information contained in another single document submitted in a separate procurement procedure;

e) information contained in the statement they are preliminarye confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure;

3. current certificate of the competent head of the tax office confirming that no valid court judgment or final administrative decision on late payment of taxes or fees has been issued against the Contractor issued no earlier than 3 months before the deadline for submitting bids;

4. current certificate of the competent ZUS or KRUS branch confirming that the Contractor has not been issued a final court judgment or final administrative decision on late payment of social or health insurance contributions issued not earlier than 3 months before the deadline for submitting bids;

5. current excerpt from the relevant register or from the central record and informationbusiness activity, if separate provisions require an entry in the register or a register, in order to demonstrate the lack of grounds for exclusion issued not earlier than 6 months before the deadline for submission of bids;

Note:

6. information in relation to belonging to a capital group:

a) The Contractor, within 3 days from placing information on the website of the opening of offers, provides the Employer with a declaration of belonging or not belonging to the same capital group. Along with submitting the statement, the Contractor may provide evidence that links with another Contractor do not lead to a distortion of competition in the procurement procedure;

b) The Employer shall attach to the specimen the Information in relation to belonging to the capital groupconstituting Annex 4 to the ToR;
