Before awarding the contract, the Contractor whose offer has been assessed the highest, will be requested to submit in a period not shorter than 10 days, documents valid as at the date of their submission: ) confirmation of the notification to the competent authority of the Trade Quality Inspection of Agricultural and Food Products, undertaking economic activityj in the scope of production, storage, confectioning and trade in agri-food products, in accordance with the Act of 21.12.2000 on the commercial quality of agricultural and food products; The activity covered by the confirmation is to relate to the part of the order or the type of food (product group) covering the subject of the order.
B) current decision or certificate of the competent authority of the State Sanit Inspectionin accordance with the Act of 25.8.2006 on food and nutrition safety regarding the entry in the register of plants or on the approval of a plant subject to official control of the State Sanitary Inspection in the production (if the Contractor is a producer) of storage, confectioning and trade in articles agri-food or storage, confectioning andbrotu agri-food products (if the Contractor is not a producer).
The activity covered by the decision is to concern the type of food (product group) covering the subject of the order. The decision or certificate is to be issued not earlier than 12 months before the deadline for submission of tenders. If the document was issued in a later period than indicated above, the Contractor is obliged tot attach to the document a statement that the information contained in the document is up-to-date and has not changed on the day of the deadline for submitting bids. c) in the case when the offer is submitted by the Contractor from the European Union, from outside the Republic of Poland, the offer should be accompanied by the current decision issued by the competent authority of the country concerned regarding the approval of the establishment with regard to the type of food (product group) covering• if the offer is submitted by a third country contractor to the offer, attach the current decision issued by the competent authority in the country to allow the plant to be manufactured and exported to the markets of other countries (including the European Union countries) in the scope of the type of food (product groups) covering the subject of the order.

3. Before awarding the contract, the Contractor whose offerrated at the highest, will be requested to submit in a period not shorter than 10 days, the following documents valid as of the date of their submission:
1) in order to monster. no grounds for exclusion The Contracting Authority requires submission of documents and statements referred to in clause 9.3.1 of the ToR - in accordance with - 5 of the Regulation. 2) to confirm the circumstances referred to in Article 25 ust. 1 point 2 Pzp:
a) then the entity authorized to control the quality of monies, that the delivered products comply with specific standards or technical specifications, and in the case where the Contractor does not have a second. - ośw. that he offered agri-food products-meet the requirements for commercial quality set out in the regulations on commercial quality and additional requirements for these articles if their fulfillment has been declared by the manufacturer
b) a. or the decision or opinion of the competent authority of the State Sanitary Inspection or the Veterinary Inspection body about exercising supervision over the application of the principles of the implemented HACCP system or rulesDobrej Praktyki Produkcji / Dobre Hygienic Practice issued no earlier than 12 months before the deadline for submission of offers. The submitted certificate should confirm that the Contractor has implemented and applies the principles of the HACCP system or applies the principles of Good Manufacturing Practice / Good Hygiene Practice, the legal basis of art. 59 and 73 of the Act of 25.8.2006 on Food Safety andnutrition. The Employer also allows the presentation of another document, ie a valid certificate of an independent entity (a certification body accredited by the Polish Center for Accreditation) confirming the implementation of the HACCP system or the principles of Good Manufacturing Practice (GMP) / Good Hygiene Practice (GHP).

4.
