Łodź, area Lodz, Lodz Province is a service for the export and treatment of medical waste generated in the area ofSP ZOZ University Hospital Hospital No. 1 for them N. 1. The subject of the order of the USK No. 1 to N. Barlicki in Ł-dź, Gm. M. Łodź, area Lodz, Lodz Province is a service for the export and utilization of medical waste generated in the area of activity of SP ZOZ University Clinical Hospital No. 1 N. Barlickiego UM in Ł-dź, about the quantities and properties specified in detail in Appendix 1A to the specification being at the same time a formoffer lector.

2. Estimated annual amount of waste 250, 000 kg.

3. Collection and utilization of waste from diagnosis, treatment and medical prophylaxis (code 18 01): 18 01 02 *, 18 0103 *, 18 01 04, 18 01 06 *. 18 01 07, 18 01 08 *, 18 01 09

4. Receiving and disposing of medical waste, hereinafter referred to as waste, will take place using means of transport belonging to the Contractor.

5. Disposal inw. medical waste will be carried out in the incinerator belonging to the Contractor in accordance with the "Statement of the place of utilization" set out in Annex 1C constituting an integral part of the SETC-5. The waste collection will take place: - from the area of SP ZOZ USK No. 1, N. Barlickiego in Ł-dź, 22 Kopcińskiego St. - daily from Monday to Friday, minimum 5 times a week at 04.00 to 07.00, additional exports afterh. 16.00,

- from the area of USK No. 1 to them N. Barlickiego in Ł-dź - ZAKŁAD PATOMORFOLOGII, ul. Pomorska 251 1 once a week during the operating hours of the Department of Pathomorphology

In emergency situations requiring immediate intervention, the implementation of tasks shall be carried out immediately on the day of submitting the waste collection.

6. Requirements for the implementation of the service in question:

a. The contractor will bezie provided the service taking into account art. 20 of the Act of 14 December 2012 on waste (Journal of Laws of 2018, item 992, as amended), which defines the "PRINCIPLE OF CLOSE." In accordance with the principle of proximity, it is prohibited to dispose of infectious waste of medical and infectious waste veterinary outside the territory of the province where they were made. b. It is allowed toany waste of medical and infectious veterinary waste in the territory of the province other than the one on which it was manufactured, in the nearest installation, in the absence of installations for neutralizing such waste in the area of a given province or when existing installations do not have spare capacity.

c. WASTE DISPOSAL METHOD: According to art. 95 paragraph 2 and 3 of the Act withOn December 14, 2012, waste of infectious medical waste is disposed of by the thermal conversion of hazardous waste in incinerators. Their disposal in co-incinerators with other waste is prohibited.

D. CONFIRMATION OF WASTE DISPOSAL "Based on Article 95 (4) and (5) of the Act of 14 December 2012 on waste, the Ordering Party requires that the waste holder, ie the Contractord

7. The service should be provided in accordance with the regulations, including in particular:

a. Act of 14 December 2012 on waste (Dz.U.2019.701);

b. Regulation of the Minister of Health of 5 October 2017 on the detailed way of dealing with medical waste (Journal of Laws of 2017, item 1975). Regulation of the Minister of the Environment of January 13, 2014. on the document confirminghim disposing of infectious medical waste or infectious veterinary waste (ie Journal of Laws of 2014, item 107, as amended); Regulation of the Minister of the Environment of 25 April 2019 regarding the model of documents used for the needs of waste registration (Journal of Laws of 2019 item 819); Act of 27 April 2001 on Environmental Protection Law (Journal of Laws of 2018, item 799, as amended); rothe ordinance of the Minister of the Environment of 9 December 2014 regarding the waste catalog (consolidated text: Journal of Laws of 2014, item 1923, as amended); Regulation of the Minister of Health on the requirements and manner of disposal of medical and veterinary waste from October 21, 2016 (Journal of Laws of 2016, item 1819); Act of 19 August 2011 on the transport of dangerous goods (i.e. Journal of Laws of 2019 item.382.). List and brief description of conditions:

1. current authorization to carry out activities related to the disposal of waste covered by the order issued by the competent authority

2. entry into the register referred to in Article 49 of the Act of 14.12.2012 on waste (Journal of Laws 2018, item 992, as amended) - the Contracting Authority will consider that the Contractor meets this condition if he provides a document confirming that the contractoris insured against civil liability in respect of the business related to the subject of the order for a guarantee sum not lower than 1, 000, 000.00 PLN (in words: one million zlotys) - LIST OF DECLARATIONS AND DOCUMENTS SUBMITTED WITH THE OFFER IN THE ELECTRONIC FORM

9. Original, obligatorybinding the entity making its resources available for the needs of the Contractor submitting the offer, if applicable.

B 1. By entering the tender the contractor is obliged to make a bid bond in the amount of PLN 18, 520.00 (say: eighteen thousand five hundred and twenty zlotys 00/100).

2. The bid bond can be contributed in the following forms:

A) In money by bank transfer to the account: BGK S.A. No. 091130 1163 0014 7138 1320 0001 with an indication: "" Wadium 35 / ZP / 2019 Export and utilization of medical waste "- by September 4, 2009 to 09:00"

B) On bank guarantees or social savings and credit union guarantees ( except that the surety must be a cash guarantee), bank and insurance guarantees, as well as sureties granted by entities referred to in art.6b par. 5 points 2 of the Act of November 9, 2000. on the establishment of the Polish Agency for Enterprise Development (Journal of Laws No. 109, item 1158, as amended) for the period of being bound by the offer, ie 60 days counting from the deadline for submitting bids. The content of the guarantee / surety should be unconditional, for every written request made by the Ordering Party within the binding period of the offer, the obligation of the Guarantor topaying the Ordering Party the full amount of the bid bond in the circumstances specified in art. 46 ust. 4a and 5 of the Public Procurement Law.

4. If the bid bond has been paid in cash, then a copy of the proof of payment of the bid bond (making a bank transfer) attached to the offer is required either original or electronically certified as true copy. The date of payment of the deposit in the form of cash is the date of recognition racbanking customer`s order.

5. The bid security paid in the manner referred to in point x.2B of the ToR should be brought via the purchasing platform. The ordering party requires the submission of a document in electronic form with the reservation that it will be signed by a qualified electronic signature by the Guarantor, ie the issuer of the guarantee / guarantee.

6. If it is not possible to pay a deposit in formNot described in point x.5 of the Terms of Reference, the Employer accepts payment of the bid bond in the form of an original guarantee / surety document.
