The subject of the order is 24-hour provision of medical services in the field of blood collection and examination of persons detained by officers Municipal Police Headquarters in Tarnobrzeg (without KP NowaDęba).

1. The subject of the order is the 24-hour provision of medical services in the field of blood collection and examination of persons detained by officers of the Municipal Police Headquarters in Tarnobrzeg (excluding KP Nowa Dęba).

2. Services constituting the subject of the contract will be provided at the Contractor`s premises in Tarnobrzeg.

3. The Employer provides the option right to this contractienia, this means the possibility of extending the number of services of the basic order by additional quantities. The number of services in the basic order and the order under the option is specified in Annex 1 to the ToR. Description of the subject of the order.

1. The offer should contain:

1) the offer form which wzor is Annex 2 to the ToR;

2) statements required by the provisions of paragraph 7 para. 1 SIWZ.

2. Offer validity period: 60 days.

3. The Awarding Entity, before awarding the order, will call the Contractor whose tender has been rated the highest to be submitted within a specified period of not less than 5 days by the date of the current fillings of the submission of statements or documents confirming the circumstances referred toand in art. 25 paragraph 1 of the Public Procurement Law, ie: an excerpt from the relevant register or from the central register and information on business activity, if separate provisions require an entry in the register or the register, in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law

4. The Contractor, within 3 days of posting on the website the information referred to in art. 86paragraph. 5 of the Public Procurement Law, provides the Employer with a declaration of belonging or not belonging to the same capital group.

5. The term of the contract: from the date of signing the contract to 31/12/2022
