The subject of public procurement is the supply of an automated digital scanner of histological preparations after digitization and evaluation of preparations for the Histomorphometric Laboratory of the Institute of Pathology Generally Faculty Hospital in Praze, including admission, customs, transport, installation, putting into operation, entrance validation, execution of instructions according to the relevant provision of Act No. 268/2014 Coll. on Medical Devices (Act No. 268/2014 Coll. training of the buyer's operator and transfer of the documents relating to the goods to be supplied, especially the declaration of conformity and instructions for use in the Czech language both in printed and electronicThe main subject of the public procurement is the supply of an automated digital scanner of histological preparations after digitization and evaluation of preparations for the Histomorphometric Laboratory of the Institute of Pathology of the General University Hospital in Prague, including the following: admission, customs, transport, installation, uvcommissioning, entry validation, execution of instructions according to the relevant provision of Act No. 268/2014 Coll. on Medical Devices (Act No. 268/2014 Coll. training of the buyer's operator and handing over the documents relating to the goods to be supplied, in particular the declaration of conformity and the instructions for use in Czech in printed and electronic form, including warranty service and others(a) by extracting from the Commercial Register, if it is entered in it, or by an extract from another similar to that in which the Contractor is registered according to a special legal regulation (Section 77 (1) of the Act); (b) proof of correction for undertaking according to special legal regulations in the scope corresponding to the subject of a public offer, especially a document proving the corresponding trade license or a lice(c) a document attesting to its fitness for distribution and servicing of medical devices, ie one of the following possibilities: - The confirmation of the registration of the registration shall be issued by the SLA or - The extract from the S-KL register (Section 77, paragraph 2, letter a) pursuant to Act No. 268/2014 Coll. or the URL that can be verified for registration

(if a medical device is available under Act No. 268/2014 Sb.)

D) proof of authorization of the manufacturer for the distribution and servicing of the subject VZ
