Medical equipments.

The subject of the orders is the delivery of medical equipment to the Department of Gynecology and Obstetrics for Głog-w Szpital Powiatowy Sp. z o.o.

The subject of the order is one-off-delivery of medical equipment.

The subject of the orders is divided into 10 parts, hereinafter referred to as the Packages.

The ordering party accepts submitting partial offers, ie to any chosen packages.

In particular, the subject of the order is described in Annex No. 3, ie List of technical parameters and conditions for implementation of the orders have been specified in the draft contract Annex No.4.

The proposed subject of the order must be brand new, manufactured at least in 2017.

The subject of the order must meet the conditions required by law to allow them to use. He must have documents allowing them to be used in healthcare facilities in the Real EstatePoland, in accordance with the requirements set out in the Act of 20.5.2010 on medical devices and secondary legislation to this Act, ie declaration of compliance with the essential CE requirements and, if required by separate regulations, a certificate of a notified body that participated in assessment of a medical device, specified in the Medical Devices Act if you do notz oo, an appropriate statement must be submitted together with the offer. • The range offered must meet the requirements of the Ordering Party given in the subject description of the order and fully meet the functional requirements as intended.

The selected Contractor undertakes as part of the future contractual provision at his own risk for the delivery of the item to the registered office of the Ordering PartyQuality contractor with regard to the subject matter of the order, the Contractor will add information materials to the offer: catalogs, brochures, leaflets, descriptions, etc.

The ordering party does not allow variant offers.

Packages No. 1, 2, 3, 4, 5, 6, 7, 8, 9 delivery by 15.6.2018. - Package No. 10 delivery by 15.11.2018.

Requirements for the bid bond.

The ordering party requires the contractors to pay a bid bond.

The offer must be secured by a bid bond.

The total amount of bid bond is PLN 37, 320.00. Package No. 1 = PLN 400.00. Package No. 2 = PLN 350.00 PLN.

Package No. 3 = PLN 700.00. Package No. 4 = PLN 400.00. Package No. 5 = PLN 180.00. Package No. 6 = PLN 1, 200.00. Package No. 7 = PLN 900.00. Package No. 8 = PLN 180.00. . Package No. 9 = PLN 20, 000.00. Package No. 10 = PLN 13, 000.00. The bid bond must be submitted before the deadline for submitting offers. The bid bond can be paid in the following forms:

• money, payable by bank transfer to: 92 1130 1033 0018 7994 5920 0003 (for the date of deposit payment considersdate and time of receipt of money on the account of the ordering party), • bank guarantees or social savings bank guarantees, however, the surety is always a cash guarantee,

bank guarantees, insurance grants, and b guarantees granted by entities, referred to in Article 6b paragraph 5 points 2 of the Act of 9.11.2000 on the establishment of Polskiej Agencja Rozwoju Przedsiębiorczości (Journal of Laws No. 109, item 1158, as amended).
