The subject of the contract is: Purchase of medical equipment and apparatus for the retrofitting of the Reumat departmentwith the subdivision of Geriatrics and the Department of Diagnostic Imaging for the purposes of combating osteoporosis as part of project No. PBU1 / 0326/16 entitled "Model program of health care against osteoporosis in the Polish-Belarusian border area" in the quantities, assortment specified in Annex 5 to the ToR and about the parameters specified in Annex 6 to the ToR.

Contractors may apply for the contract who are not subject to exclusion pursuant to art. 24 paragraph 1 points 12-23, art. 24 paragraph 5 point 1, art. 24 paragraph 5 point 8 mouthPPL and meet the conditions for participation in the procedure - the Employer does not specify the conditions for participation in this procedure. 3. Contractor who is subject to exclusion pursuant to art. 24 paragraph 1 points 13 and 14and 16-20 or paragraph 5, may provide evidence that the measures taken by him are sufficient to demonstrate its reliability, in particular to prove compensation for damage caused by a crime or tax offense, monetary compensation for harm suffered or compensation, exhausting an explanation of the state actual and cooperation with law enforcement agencies and undertaking specifictechnical, organizational and staffing measures that are appropriate to prevent further offenses or fiscal offenses or misconduct by the contractor. The first sentence shall not apply if the contractor, who is a collective entity, has been ordered by a final judgment of a court to prohibit applying for the contract and has not expired specified inyroku duration of this prohibition.
4. The Contractor is not subject to exclusion if the Employer, taking into account the weight and special circumstances of the Contractor's act, deems sufficient evidence provided pursuant to point 3. In the cases referred to in art. 24 paragraph 1 point 19, before excluding the Contractor, the Employer shall provide the Contractor with the opportunity to prove that his participation in the preparation of posordering will not distort competition.
6. The required documents (submitted by the Contractor along with the offer or at the request of the Employer) are specified in chapter VII of the ToR. 7. Requirements for the bid bond were specified in chapter IX of the ToR. Amendments to the provisions of contracts concluded as a result of these proceedings may be introduced in the circumstances and in the manner indicated in zAnnex 2 to the ToR (Contract template)
