1 The subject of the order is the purchase, delivery, assembly and commissioning of medical equipment and medical equipment for the Division Urological Hospitalla voivodship in Łomża, in quantities, assortment and technical and required parameters specified in Annex 4 and Annex 5 to the ToR.

The subject of the orders is the purchase, delivery, assembly and commissioning of medical equipment and medical equipment for the Urologic Divisionego of the Voivodship Hospital Cardinal Stefan Wyszyński in Łomża, in quantities, assortment and technical and required parameters specified in Annex No. 4 and Annex 5 to the ToR.

5. Mini Nephroscope - 1 item. The terms of the order are defined in the contract form for Zaconnector No. 6 to the ToR.

The Employer does not specify the conditions for participation in the procedure. • The Contracting Party does not specify the conditions for participation in the proceedings. The exact date of the offer binding is 60 since the deadline for submission of offers.

2. The bid bond for the entire order is PLN 14, 100.00.

3. Contractors who are not subject to exclusion in accordance with art. 24 sec. 1 and 24 para. 5point 1) of the Public Procurement Law and meet the conditions for participation in the proceedings.

4. The Contractor submits together with the offer a statement in the form of EAT (Annex No. 3 to the Terms of Reference).

5. In addition, the Contractor submits along with the offer:

1) Bid form (according to Annex 1 to the ToR),

2) List of assortment and quantity and technical parameters and required for the offered medical equipment and medical equipment (according to AnnexNo. 4 to SIWZ),

3) Form "Warranty and post-warranty service, training" (according to Annex No. 5 to the ToR),

4) Power of attorney (if applicable).

6. Contractor, within 3 days of posting on the website information referred to in Article 86 (5) of the Public Procurement Law, shall provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to inArt. 24 sec. 1 point 23 of the Public Procurement Law (Appendix 2 to the Terms of Reference).

7. The contracting authority will apply the procedure referred to in art. 24aa paragraph 1 of the Public Procurement Law (the reverse procedure).

8. Based on Article. 26 section 1 of the Public Procurement Law, the Contracting Authority will call the Contractor whose tender was the highest rated, to submit documents and statements confirming the lack of grounds for exclusion from the proceedings and confirming the fulfillment ofgiven by the offered deliveries Ordering Party requirements specified in SIWZ, listed in Chapter VI lit. C SIWZ.

9. Contractors may jointly apply for granting a public order on the terms set out in art. 23 Public Procurement Law.

10. Contractors jointly applying for the award of the contract establish a representative to represent them in the procurement procedure or representationsin the proceedings and conclusion of a public procurement contract.

11. The information clause concerning the GDPR is included in Chapter XIX of the Terms of Reference.

12. Communication between the Ordering Party and the Contractors, in particular submitting bids and statements, including a declaration submitted on the Uniform European Order Document (EAT) form, is carried out using the medium of communicationktronicznej.
