2. The detailed description of the subject of the order, the required parameters and general requirements for the subject of the order are included in Annex 2 concerning the description of the subject of the order, which is an integral part of this SIWZ.

Set of laboratory accessories

Orderingand contractors who can not be excluded due to the lack of grounds for exclusion due to failure to meet the conditions referred to in Art. 24 section 1 items 12-23 of the Act. The Employer does not provide for the possibility of excluding the Contractor based on Art. 24 sec. 5 of the Act

II. In order to confirm the absence of grounds for exclusion of the Contractor from participation in the proceedings, the Contracting Authority requires submission of the followingpending statements or documents:

1) Contractor`s declaration submitted on the basis of 25a para. 1 of the Public Procurement Law, regarding the conditions for exclusion from proceedings, in the form of a Single European Order Document according to Annex 4 to the ToR.

2) Information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Act issued not earlier than 6 months before the deadlineat the submission of offers. 3) Declaration of the Contractor about the lack of issuing to him a final court judgment or final administrative decision on defaulting taxes, fees or social security or health insurance premiums or in the event of issuing such a decision or decision documents confirming the payment of these claims together with any interest or fines, orb conclusion of a binding agreement regarding the repayment of such receivables,

4) Declaration of the Contractor about the lack of a ruling against him as a preventive measure of the prohibition to apply for a public order,

5) Contractor`s declaration of affiliation or lack of belonging to the same capital group; in the case of belonging to the same capital group, the contractor may submit together with the statementm documents or information confirming that connections with another contractor do not lead to distortion of competition in the proceeding within 3 days of posting information on the website referred to in art. 86 par. 5 Public Procurement Law - VI. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the document referred to in point II (2), he shall submit information from the counterpartyedny register, or in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence has a person who, for information or a document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Act

VII. If in the country in which the Contractor is established or placese) the person concerned by the document does not appear, the document referred to in point VI is not replaced by a document containing the Contractor`s declaration, indicating the person or persons authorized to represent him or the statement of the person to whom the document was to apply, submitted to a notary or before a judicial, administrative or administrative bodya professional or economic self-governing authority with regard to the seat or place of residence of the Contractor or place of residence of that person.

VIII. The Contractor established on the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to which the document referred to in point II (2) applies, submits the document referred to in point VI within the scope defined in Article 24 paragraph 1 pkt 14 and 21 acts. If the country in which the person has a document to reside is located, no such documents are issued, it is replaced by a document containing a statement made by the person before a notary or before a judicial, administrative or self-regulatory or economic self-regulatory authority residence of that person. Documents aboutmentioned in items VII, VII and VIII must be issued not earlier than 6 months before the deadline for submission of tenders. In order to confirm that deliveries meet the requirements of the Ordering Party, one should submit: information materials; in the case of equivalent solutions - a declaration on the equivalent offer (provide documents, ie Polish and European approvals, certificates).
