The subject of the public procurement is to ensure the supply of equipment in connection with the reconstruction of the internal ICU, namely:

Medical Devices, Main Site: Třinec Hospital, contributing organization, 739 61 Třinec - Dolni Lištna, Kaštanova 268, CZECH REPUBLIC.the principal part of this part is an infusion set supply for the needs of interni ICUs. The infusion set consists of 1 piece of docking station, 4 pieces of line hoppers and 2 infusion pumps.

The fulfillment of professional competence according to - 77 proves the participant of the introductory procedure, who shall submit:

a) a simple copy of the commercial listing, if it is entered in it, or a statement from another similarly to the record, if registered in it (- 77 para(b) a simple copy of the document on the right of establishment (for example, extract from the trade register) according to the special legal regulations in the extent corresponding to the subject of the public offer, especially a document proving the trade license or other correction (- 77 par. ) The sponsor determines that the qualification requirement is fulfilled to the minimum extent necessary to fulfill this requirementincluding the following: • Production, trade and services not listed in Appendices 1 to 3 of the Trades Licensing Act.
